Dear Robert Schuetz:

On October 7, 2024, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Columbia Generating Station and discussed the results of this inspection with Grover Hettel, Executive Vice President/Chief Nuclear Officer and other members of your staff. The results of this inspection are documented in the enclosed report.

No NRC-identified or self-revealed findings were identified during this inspection.

A licensee-identified violation which was determined to be of very low safety significance is documented in this report. We are treating this violation as a non-cited violation (NCV) consistent with Section 2.3.2 of the Enforcement Policy.

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting a fire protection team inspection at Columbia Generating Station, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. 2 INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

REACTOR SAFETY

71111.21N.05 - Fire Protection Team Inspection (FPTI)

Structures, Systems, and Components (SSCs) Credited for Fire Prevention, Detection, Suppression, or Post-Fire Safe Shutdown Review (IP Section 03.01) (6 Samples)

The inspectors verified that components and/or systems will function as required to support the credited functions stated for each sample. Additional inspection considerations are located in the fire hazards analysis (FHA) or safe shutdown analysis (SSA).

(1) High Pressure Core Spray (HPCS), DG-1 (2) Cable Spreading Room, RC-2 (3) Reactor Building - General Equipment Area El. 422, R-1EL 422 (4) Main Control Room, Non-abandonment, RC-10 (5) Reactor Building El. 522, R1-E522 (6) Inspected Probabilistic Risk Assessment (PRA) Configuration Control (PCC) using the Operating Experience Smart Sample (OpESS) 2023-02 to verify that the licensee has a process in place to reasonably ensure PCC activities.

Fire Protection Program Administrative Controls (IP Section 03.02) (2 Samples)

The inspectors verified that the selected control or process is implemented in accordance with the licensees current licensing basis. If applicable, ensure that the licensees fire protection program contains adequate procedures to implement the selected administrative control. Verify that the selected administrative control meets the requirements of all committed industry standards.

Fire Protection Program Changes/Modifications (IP Section 03.03) (7 Samples)

The inspectors verified the following:

a. Changes to the approved fire protection program do not constitute an adverse effect on the ability to safely shutdown.
b. The adequacy of the design modification, if applicable.
c. Assumptions and performance capability stated in the safe shutdown analysis have not been degraded through changes or modifications.

d. The fire protection program documents, such as the updated final safety analysis report, fire protection report, fire hazard analysis, and safe shutdown analysis were updated consistent with the fire protection program or design change.
e. Post-fire safe shutdown operating procedures, such as abnormal operating procedures, affected by the modification were updated.

INSPECTION RESULTS

Licensee-Identified Non-Cited Violation 71111.21N.05 This violation of very low safety significance was identified by the licensee and has been entered into the licensee corrective action program and is being treated as a non-cited violation, consistent with Section 2.3.2 of the Enforcement Policy.

Violation: Facility Operating License (FOL) Condition 2.c.14, "Fire Protection Program,"

specified that Energy Northwest, shall implement and maintain in effect all provisions of the approved fire protection program as described in Section 9.5.1 and Appendix F of the Final Safety Analysis Report (FSAR). Specifically, FSAR Section 13.2.2.5.2 Offsite Fire Department, states in part that, the offsite fire department that supplements the Fire Brigade will attend familiarization training associated with the Columbia Generating Station plant layout, operational precautions, radiation protection, and special hazards associated with fires at a nuclear power plant. This offsite fire department will participate in at least one fire drill each year.

Contrary to the above, the licensee determined that the offsite fire department did not participate in at least one fire drill each year and a violation of their license condition had occurred. Specifically, the licensee documented on December 14, 2024, in action request (AR) 453566 multiple attempts to conduct cooperative drill exercises with Hanford Fire Department in 2023 and 2024. The licensee generated an action request to address the drill cancellations, to schedule and conduct the annual fire drill with the off-site department in accordance with EPlan, to make changes to the EPlan, and to completes a drill scheduled for September 26, 2024. Additionally, the licensee engaged legal to clarify the duties and expectation associated with the interagency agreement with Hanford Fire Department.

Significance/Severity: Green. This deficiency was more than minor because the finding is associated with the Protection Against External Factors attribute of the Mitigating Systems Cornerstone since it could affect the availability, reliability, and capability of systems that respond to a fire event to prevent undesirable consequences. The inspectors processed this finding using the guidance of Inspection Manual Chapter 0609, appendix F, Fire Protection Significance Determination Process. The inspectors determined from step 1.2, Assign a Fire Finding Category, that the fire prevention and administrative controls was the best category associated with the training programs element for the deficiency. From appendix F,

4 attachment 2, Degradation Rating Guidance, the inspectors determined the deficiency had a low degradation since it was similar to the examples of low degradation and would not have a significant effect on the likelihood that a fire would occur. Based on the criteria in Attachment 2, the inspectors assigned a Low degradation rating and the fire protection finding screened to green (Yes to question 1.3.1-A).
