AFFIDAVIT OF PATIY S. MIRmm County of diarlestm State of S. Carolina Patty S. Miriello, being duly sworn according to law, deposes and says as follows:

and at the Brunswick Stasen Electric Plant (BSEP is a two reactor plant site),

2. 'Ihm purpose of the affidavit is to show that the issues of: false dose records, intimidation, and unsafe radiatim practices at CP&L nelaar plants was brought to the attention of the USNRC and that there were no actions identified in scrne of these natters until-after contention W-4-wes-prMvwi.

By January 1985 it as determined fran these splayees that there were problesns with the records which already showed a dose far below that: indicated by survey instrunents and stay time / dose cairnlation. mreover, this dose as insignifi-cant derison to the pocket dosimeter raadings.

In Jarmary of 1986, all of the natters di-sed above were regurumil to USNRC inspm.u.u.
