AFFIDAVIT OF PATIY S. MIRmm County of diarlestm State of S. Carolina Patty S. Miriello, being duly sworn according to law, deposes and says as follows:

27611. I was employed by Carolina Power & Light frun February 1985 through August 1985. I worked at the Shearon Harris Nuclear Power Plant (SIBIPP), New Hill, N.C.

and at the Brunswick Stasen Electric Plant (BSEP is a two reactor plant site),

A.Cwi., N.C. I ms employed as a health physws technician and worked in radiation areas and was exposed to radiation and radinactive contmaipation in,the course of my work at both plants.

2. 'Ihm purpose of the affidavit is to show that the issues of: false dose records, intimidation, and unsafe radiatim practices at CP&L nelaar plants was brought to the attention of the USNRC and that there were no actions identified in scrne of these natters until-after contention W-4-wes-prMvwi.

In SwM 1985 the Office of Investigation of the United States Nuclear Regulatory hianion - Atlanta was notified of unsafe radiation pract-im, does reu.u.ds that didn't agree with the exposure Wiich was noted on several instrunents by myself and another technician Wille that technician was showing me problemn radiation areas Much were not properly posted and wh.t.le we surveyed the areas, and ineimidation and surveil-lance by CP&L health physics per .ml.

IN Noverter 1985 crritact as made with two CP&L splayees to check on the false dose records. By January 1985 it as determined fran these splayees that there were problesns with the records which already showed a dose far below that: indicated by survey instrunents and stay time / dose cairnlation. mreover, this dose as insignifi-cant derison to the pocket dosimeter raadings.

In Jarmary of 1986, all of the natters di-sed above were regurumil to USNRC inspm.u.u.
