REBUILT DIESEL GENERATOR TEST FAILURE.

On June 5,1987, the "B" Diesel Generator failed to start during its second post maintenance test run since a connecting rod failure in December, 1986. Operators in the area reported hearing a popping sound from the engine after receipt of the start signal.

Licensee investigation identified a through wall crack in both the 8R cylinder head and piston liner. The cylinder head was the original; however, the piston liner was replaced following the December, 1986 failure. Cause of the damage is under review by the licensee and the diesel supplier, Cooper Energy Services. The piston liner has been replaced and preparations are being made to install a spare cylinder head. The diesel had run successfully unloaded for 15 minutes several days earlier. The resident inspector is monitoring the licensee's investigation into the cause of the engine failure.
