APPENDIX A NOTICE OF VIOLATION Yankee Atomic Electric Company Docket No. 50-29 Yankee Nuclear Power Station License No. DPR-3 l

As a result of the inspection conducted during the period May 11-29, 1987, and in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement Policy) (1987),

the following violation was identified:

License No. DPR-3, Appendix A, Technical Specifications, Section 4.4.10.4(b),

Surveillance Requirements, states, in part, that the steam generators shall be determined operable after completing the corresponding actions required during sample tube inspections. One such action includes plugging all tubes exceeding the defect plugging limit of greater than 40% through wall.

Contrary to the above, during the 1984 refueling outage, the licensee failed to plug all steam generator tubes exceeding the Technical Specifi-cation plugging limit.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Yankee Atomic Electric Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
