APPENDIX A NOTICE OF VIOLATION Gulf States Utilities Docket: 50-458 River Bend Station License: NPF-47 l

During an NRC inspection conducted on September 16 through September 30, 1987, two violations of NRC requirements were identified. The violations involved a

' failure to utilize proper revision of surveillance test procedure and an inadequate station operating procedure. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violations are listed below:

A. Inadequate Station Operating Procedure Technical Specification 6.8.1.a. requires that written procedures be established, implemented and maintained for the fuel storage pool purification and cooling system. The licensee's approved quality assurance program requires that such procedures be reviewed to assure adequacy.

Contrary to the above, the required review of Station Operating Procedure 50P-0091, " Fuel Pool Cooling & Cleanup," did not assure adequacy in that strict administrative control of fuel pool anti-siphon plugs was not provided and Section 5.2.2 provided a valve line up which would establish a siphon path from the upper fuel storage pool to the condensate storage tank (CST).

This is a Severity Level IV violation. (Supplement I)(458/8724-01)

B. Failure to Utilize Proper Revision of Surveillance Test Procedure Technical Specification 6.8.1.g requires that written procedures be established, implemented and maintained for activities implementing the fire protection program.

Administrative Procedure (ADM)-0015, " Station Surveillance Test Program,"

requires that an individual assigned to perform a surveillance test utilize an official work copy of the latest test revision of the surveillance test procedure (STP) and any associated temporary change notices.

Contrary to the above, STP-000-3001, " Daily Fire Door Position Check,"

performed on September 20, 1987, and September 22, 1987, did not utilize the latest revision of the STP which was available from station document control on August 27, 1987.
