i a NOTICE OF VIOLATION Illinois Power Company License No. NPF-55 As a result of the inspection conducted on February 24 through April 6,1987, and in accordance with the " General Policy and Procedure for NRC Enforcement Actions", 10CFR Part 2, Appendix C (1985), the following violation was identified:

Clinton Technical Specification 6.8.1 requires that written procedures be established, implemented, and maintained concerning activities referenced in Appendix A of Regulatory Guide 1.33 and for surveillance and test activities of safety related equipment.

Administrative Procedure CPS No. 1401.01, Conduct of Operations, Revision 11, paragraph 8.4.3.2.f required the status of any current j Limiting Condition for Operation (LCO) be conveyed at shift relief.

! Administrative Procedure CPS No. 1401.01, Conduct of Operations, Revision 11, paragraph 8.4.1.3 required that in other than emergency situations, when a procedural step cannot be completed as stated, the procedures shall be changed per CPS No. 1005.07, Temporary Changes to Station Procedures, prior to work continuing.

Plant Manager Standing Order No. 30 required that prior to performance of a mode 1, 2, and 3 surveillance an " impact matrix" be prepared to evaluate the affect of that surveillance on plant systems.

Contrary to the above:

, 1. On February 27, 1987, day shift and swing shift control room operators failed to convey the status of out of specification differential pressure readings they had recorded during the conduct of surveillance CPS No. 9000.01D001, " Control Room Operator Surveillance Log - Mode 1, 2, 3 Data Sheet".

2. On March 28, 1987, during exercising of the Traversing In-Core Probes per CPS No. 3322.01, the on duty shift supervisor made a decision to allow the work to continue after finding that the procedure did not provide sufficient instruction in step 8.1.2.12 to allow the operator to get to the next step without additional manipulation of the equipment.

This was done without making a change to the procedure to insert the additional step that was required to be taken.

3. On March 30, 1987, personnel performing surveillance procedure CPS No. 9030.01C035, revision 20, failed to implement the instructions contained in Plant Manager Standing Order No. 30. This failure resulted in an unnecessary ESF actuation when the Reactor Core Isolation Cooling system isolated.

Notice of Violation 2 These failures to implement the requirements of the applicable procedures are a Severity Level IV violation (Supplement I) (461/87011-03(DRP)).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.
