APPENDIX A NOTICE OF VIOLATION Gulf States Utilitiet, Docket: 50-458/85-77 River Bend Station License: NPF-47 During an NRC inspection conducted during the period of October 31 through November 30, 1985, one violation of NRC requirements was identified. The violation involved the failure to provide clear and concise design instructions on an approved change to a safety-related design modification. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed below:

Failure'of Design Control Program 10 CFR Part 50, Appendix B, Criterion V, and the licensee's approved quality assurance program require that activities affecting the quality of safety-related systems and components be prescribed by and conducted

-in accordance with documented instructions, procedures, or drawings, of a type appropriate to the circumstances.

Contrary to the above, on November 8,1985, it was discovered that a 96-page Field Change Notice (FCN19) to Design Modification Request (MR) 85-0397 was not appropriate to the circumstances in that it provided design instructions that were misleading and difficult to follow. For example, page 36 of FCN-19 added contacts on terminal board 43B (contacts 7/8 and 3/4) contacts on an electrical schematic for remote shutdown operation of the diesel generator output brea' r and page 57 of FCN-19 removes the same contacts. Also, several pa,. ,t FCN-19 were found to be illegible due to poor reproduction quality.

This is a Severity Level IV violation (Suppleent I.D.)(458/8577-01).

- Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submit to the Office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. When good cause is shown, consideration will be given to extending the response time.
