REQUEST FOR CORRECTIONS TO LICENSE AMENDMENT NO.193 (TAC NO. MA4131)

Dear Mr. Mellor:

We have received your letter dated October 30,1998, requesting corrected pages to be issued for License Amendment No.193, which was issued on June 30,1998.

In order to establish that a typographical error discovered in the Technical Specifications (TS) can be processed as a correction, a licensee must establish that (1) the error was -

introduced in a particular license amendment, and (2) the erroneous change was not noticed or evaluated by the staff. Under these conditions, an error can be corrected by a letter from NRC to the licensee.

The staff has reviewed your request to determine if you meet the conditions for issuing corrections. The corrected pages enclosed with your October 30th letter were compared to the changes requested by you in your license amendment application dated May 30, 1997, which was the basis for License Amendment No.193. The staff found that some of the corrections requested were not described in your October 30th letter.

The staff noted that several of the corrected pages enclosed in your October 30th letter have inserted change bars that do not ar. pear on pages issued with License Amendment No.193. The affected pages are 3/4 9-2, 3/4 9-3, 3/4 9-3a, 3/4 9-7, 3/4 9-12, and 3/4 9-15 through 3/4 9-20. However, these corrections have not been identified with a particular license amendment and no verification has been included to show that removing the bars was not noticed and evaluated by the staff. Please send a revised description of the change bar corrections that includes this information.

The staff noted that the corrected pages sent in your October 30th letter include a date in the upper right hand corner and that a reference number has been added or changed in the i lower left hand corner. Based on discussions with your staff, the page dates and reference i numbers are for your internal tracking processes and are not intended to be a part of the TS. If that is the case, please include a statement to that effect in your response.

The staff notes that the footnote on page 3/4 9-3, as submitted for correction in your October 30th letter, now has two asterisks rather than one. Please determine what is correct for this page and revise your correction request accordingly. II 9812310179 98122123

Please identify when the error was introduced, verify that it was not noticed and evaluated by the staff, and include this information in your revised description of corrections to be made.

Page 3/4 9-7, as submitted in your October 30th letter, corrects a typographical error in the amendment number on the page. However, the error on page 3/4 9-7 was not introduced in License Amendment No.193, therefore, you need to identify when it was introduced and verify that it was not noticed and evaluated by the staff. Please include this information in your revised description of corrections to be made.
