APPENDIX A NOTICE OF DEVIATION Gulf States Utilities Docket: 50-458 River Bend Station License: NPF-47 Based on the results of an NRC inspection conducted on April 28 through May 2, 1986, a deviation from the River Bend Station Environmental Qualification Document, Revision 1, March 1985, provided in accordance with 10 CFR 50.49 was identified. The deviation consisted of a failure to maintain current equipment qualification documentation. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Action," 10 CFR Part 2, Appendix C (1985), the deviation is listed below:

Section 5.1 of the River Bend Station Environmental Qualification Document, Revision 1, states that the master list of all equipment within the scope of the environmental qualification program is provided, and Section 5.2 states that System Component Evaluation Work (SCEW) sheets present a description of the individual equipment and its location.

In deviation from the above, the master list and the SCEW sheets for SRN 228214-2 listed operator model number SMC (an unqualified operator) in 12 different locations (1821*MOVF027A, 027B, 027C, 027D, ILSV*MOV11A, 11B, 13A, 13B, 15A, ISB, 16A and 168), and SCEW sheets also did not reference wiring qualification reports which are a part of the qualifica-tion package. Gulf States Utilities is hereby requested to submit to this office, within 30 days of the date of this letter transmitting this Notice, a written statement or explanation in reply, including for each deviation: (1) the reason for the deviation if admitted (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further deviations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
