J.C. 20555 with a cocy to the Regional Administrator, Region IV, anc if acolicacie, a ::oy to the NRC Resicent Inspector, within 30 days of :ne cate of the 1e::er transmitting this Notice of Violation (Notice). This reply snould be clearly marked as a " Reply to a Notice of Violation" ano snould incluce for eacn violation: (1) the reason for the violation, or, i# contestec, ne casis for discuting tne violation, (2) the corrective steos tnat have oeen taken ana :ne results acnieved, (3)' the corrective steps that will be taken to avoic furtner violations, and (4) the date when full compliance will be acnievea. :f an adeouate reply is not received within the time specified in this Notice, an i orcer may oe issued to show cause why the license snould not ce moaified, suspenced, or revoked, or why such other action as may be proper snould not ce taKen. Where good cause is shown, consideration will be given to extencing the response time. Under the authority of Section 182 of the Act. 42 U.S.C. :32, this resconse shall be submittec under oath or affirmation.
