Gentlemen:

SUBJECT: NRC INSPECTION REPORT NO. 50-458/91-09 (NOTICE OF VIOLATION)

This refers to the insoection conaucted by Messrs. R. Calcwell anc 3. Earnest

of this office on February 25-26, 1991. The insoection inciuced a review of

activities authorizea for your River Bena Station facility. At the conclusion

of the insoection, the findings were discussea witn those :emoers of your staff

identified in the enclosed report.

Areas examined during the inspection are identifiec in tne report. Within

hese areas, the inspection consisted of selective examination of crocecures

Tna representative records, interviews with cersonnel, ana 00servation Of

octivities in progress.

Basea on the results of this inscection, one of your activities acoearea tc ce

in violation of NRC requirements as specifiec in :ne enciosec Notice of

Violation (NOV). We are concerneo aoout the violation because you reaucea :ne

size of the security organization without firs: cnanging the croceaures anc

policies necessary to prevent a degracation of :ne effectiveness of :ne

security organization.

On February 3, 1991, after reducing ne size of :ne security organi:ation i,

January, your quality assurance organization issueo a recort icentifying

inadeouate security snift staffing required to satisfy ne reouirements of

Procedure PSP-4-409. On February 25, Region IV insoectors ceterminea :nat

security shift staffing had not been added to meet these reouirements.

Although your organization identified this proolem, corrective actions were not

initiated to address the staffing inaceauacies until Marcn 7, 1991, anen

memoers of your staff stated teleononically that precedure cnanges were to ce

issued Marcn 8, 1991, and all applicable RBS security personnel would be

exercised on this procedure by Maren 13, 1991. As a result of the failure to

promptly resolve this issue, we believe that there was aiminished effectiveness

in the security program because of this untimely cnange in proceaure ana lack

of training in the execution of the changed procedure.

You are reauired to respond to this letter and snould follow the instructions

specified in the enclosed Notice when precaring your resconse. In your

response, you should document the specific actions taken ano any accitional

Notice, including your proposed corrective actions and the results of future

insoections, the NRC will determine whetner further NRC enforcement action is

necessary to ensure compliance with NRC regulatory reouirements.

One violation identified in paragraph 7 is not being cited in accoraance witn

the criteria of Section V.A of NRC's Enforcement Policy because of tne minor

significance of the violation wnich involved visitor access control.

We nave also examined actions you have taken with regard to a previously

identified insoection finding. The status of tnis item is identified in

paragraph 2 of the enclosec report.
