t APPENDIX A NOTICE OF VIOLATION L '

Gulf States Utilities. Docket: 50-458/85

  • P,iver Bend Station License: NPF-47

. During~an NRC inspection conducted during the period of December 1 through 31,

,1985, two violations of NRC requirements were identified. The violations

.3 involved the failure to follow an administrative procedure for control and use 1 of procedures and inadequate retest following a modification or repair. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," .10 CFR Part 2, Appendix C (1985), the violations are listed below:

, quality assurance program require that activities affecting the quality of safety-related systems and components be prescribed by and conducted in accordance with documented instructions, procedures, or drawings, of a type appropriate to the circumstances.

GSU Administrative Procedure ADM-003, " Development Control and Use of -

Procedures," Revision 7, paragraph 6.6.6.3 requires that the control room shift clerk place a copy of the TCN (Temporary Change Notice) in' front of the affected controlled procedure in the control room Station Operating Manual (SOM) and record the TCH number on the approved page.

Contrary to the above, on December 2, 1985, the NRC inspector noted that TCN 85-1287 to Station Operating Procedure SOP-0035, Revision 0, " Reactor Core Isolation Cooling," was not included in the front of the above S0P package, located in the main control area, as required. The TCN removed laters'in the procedure, corrected a mistake in the electrical line up,

. and added valves to the valve line up sheet.

This is a Severity Level V violation (Supplement I.E.)(458/8581-01).

B. Inadequate Retest Following a Modification or Repair r 10 CFR Part 50, Appendix B, Criterion XI, requires that a test program be

. established to assure that all testing required to demonstrate that '

structures, systems, and components will perform satisfactorily in service is identified and performed.

GSU Quality Assurance Directive QAD-11, " Test Control," Revision 4, requires that tests performed following plant nodifications, repairs, or replacements be conducted in accordance with the original design and testing requirements, or a documented engineering-approved alternative and that testing be sufficient to demonstrate the acceptability of the modification or change and that the safety of operations is not reJuced.

r Contrary to the above, on December 2,1985, a review of plant condition reports (8505-0559 and 8505-0561) revealed that procedures or instructions had not provided for adequate retest following plant modifications / repairs made on two separate occasions. These were the change out of wiring to the back up scram valves with no documented continuity checks or functional test performed and a wiring change to the shutdown cooling suction valve (1E12*F008) with no valve stroking performed upon completion of wiring modifications. In both instances, no documented engineering-approved alternative testing was provided, and the function of the components was comprcmised by the modification / repairs.

This is a Severity Level IV violation (Supplement I.D.)(458/8581-02).

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violations if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
