3.1 DefLnitions 3.1.1 Design Changes --free-BELOW 3.1.1.1 Plant Modification - A planned change to the Form, Fit, or Function of any c,omponent or system for PVNGS and as required by Technical Specifications 6.5.2.3, requires Plant Manager approval. It is implemented by one of the following:

A. Design Change Package, (DCP) - A change which involves the addition, deletion, or functional design modification of any component, system or structure, where the design basis is changed. The DCP also may be used to make design changes when design basis is not changed. Design Changes are initiated through the Plant Change-Request (PCR) program and include

Engineering Eval'uated Modifications - A change that does modify the functional operation of any component, system, or structure and does require an engineering evaluation to verify that the change will-not change the design bases of the facility. Changes in this category are initiated through an identified Nonconforming Condition (EER/NC) or an EER documenting a design deviation (for non quality-related equipment).

4.0 RESPONSIBILITIES 4.1 Initiator is responsible to:

_ NOTE The STA should only be used on backshif ts or weekends where the EER must be initiated or resolved prior to the next working day.

4.1.11 Contact the S.E. or STA prior to EER initiation.  ;

4.1.12 Obtain Dept. Manager approval unauthori::ed deviations for all EER's documenting from approved plant documents. This includes such things as missed hold points in work ord ers or missed procedure steps in surveillance procedure.

4.2 Initiator's immediate supervisor is responsible for:

4.6.3 Ensuring appropriate cross-discipline review as required.

4.6.4 Ensuring that the disposition addresses the problem statement, that the disposition is the appropriate action for the identified problem, and that other alternative actions have been considered.

( 4.6.5 Ensure Interim Resolution is utilized appropriaticy.

4.7 Evaluator is responsible for:

4.7.1 Providing a disposition including all supporting documentation needed to support the resolution when requested by formal transferal of the EER to that department.

4.7.2 Provide the disposition to EERs as assigned.

4.7.-3 Obtain cross-discipline review as appropriate for any EER in which the identified deficiency or disposition involves multiple discipline type issues or involves interfacing systems, outside the evaluators normal responsibilities.

4.7.4 Return the completed EER to the EER coordinator upon completion.

4.8 EED/0CS Supervisor or designee is responsible.for:

5.4.1.5 If the Evaluator is someone other than the System Engineer, the EER shall be routed to the System Engineer via the EER Coordinator for concurrence.

5.4.1.6 If the Evaluator was the System Engineer, the System Engineer concurrence block should be marked N/A or signed and the EER processing routed to the EER Coordinator for final and closure.

5.4.2 The System Engineer shall:

5.4.2.1 Follow Section 5.4.1 for all dispositions in which the System Engineer was the Evaluator.
