Inoperable 100 ft. Elevation Air Lock Exterior Door CONDITIONS PRIOR TO OCCURRENCE:

Hot Standby: Mode 3 DESCRIPTION OF OCCURRENCE:

At 0920 hours0.0106 days <br />0.256 hours <br />0.00152 weeks <br />3.5006e-4 months <br />, during normal containment personnel entry through the 100 ft. elevation airlock, the exterior door would not close. The inner door was tagged closed and Action Statement for LCO 3.6.1.3 was initiated. At 1445 hours0.0167 days <br />0.401 hours <br />0.00239 weeks <br />5.498225e-4 months <br />, maintenance personnel had repaired the interlock device and the airlock was returned to operable status.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Failure of a plug weld on an interlock pin caused the interlock system to not allow the exterior door to close. Inner door remained closed throughout this event.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.1.3 states that with an airlock inoper-able, restore the airlock to operable status within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> or be in at least Hot Standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in Cold Shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />. The elevation 100 ft. exterior door was returned to operable status within 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br />. The unit remained in Mode 3 throughout this period.

CORRECTIVE ACTION:

The broken plug weld on the interlock pin was reworked and the interlock mechanism was reset. Air Lock door was then tested satisfactory.
