Gn June 5, i969',, I had a cal l f r om Nr. Phil Bar nes of the 8 t. Lucie 82 Nuclear Plant (407-468-4236) . He e>!plained that they last. a bearincl an the radiator shaft. The Florida Power and Light analysis concluded that the bear ing f~iled because the fit. an the shaft, was tao loose. The component was designed and furnished by the 0 5 N Manufacturing Company of Hauston > Te'>>as as part nf the r adia tar assembly .

The radiatar fan shaft was ~" raund "tock>> I discu sed this with Nr. Rich Rauner of the 6 5 N Manufacturing Company etio bel.ieved it was round stack (Tur"ned) . I fnund that 3" raund stock (Tl!rned ) lied a tolerance of +000/-004>> The bearings used are Link-Belt Pillnw Black P/N PE-822448H. l'he shaft taler anc e r equired for a 5" shaf t is +.000/-.00k. To meet this tnler.ance, ~-.l/8" diameter round stack is required fnr machining ta the proper tolerance. I have developed a sketch of the replacement. shaft 6002BK-1.

I discussed this fut ther with Phil Barnes in the late afternnan of June 6, 1989. Florida Power and Light has taken carl"ective action and replaced the fan shaf t with one of the proper tolerance.

I tald Nr. Phil Barnea that by definitinn, this defect is reportable in accordance with iOCFR2i because the failure nf t'e radiator to cool cauld cause the diesel-generator ta fail ta per farm its safety function. I asked if Flarida Power and Light would file the repnr t ~ He was waiting fnr my input. and felt that Florida Power and Light wnuld file.

I asked him ta cnnfir m this to me in writ.ing.

Ne have no other radiatol drives in this ty pe canstructian in Nuclear Plants. The END 999 type units have some that are radiator cnaled, but their design eisa is very di.f ferent. Ta my knowledge, this defect limited ta the St. Lucie Pl ar t.

Dear Mr. Rouner:

The radiator shaft bearings failed. Florida power g Light Company analysis of the failure attributed the failure to loose bearing collar in the set fit.screwcrack A

developed under the locking I believed the 3" diameter shaft was cold rolled turned stock with a tolerance of +.000 to -.004. If it was ground stock, the tolerance was +.000 -.002. The bearing PE-B22448 (3") requires a shaft tolerance of 3" +.000 to

-.001. To get this a 3-1/8 round bar would have to be used then machined to 3.000+000/-.001 diameter.

I note that the 27/5N dimension on drawings 10179 and 10180 should be 26I/5N to the center line of the fan. The 12N end dimension should be 13".

Since FPSL plans to order replacement shafts, we require verification of our findings or corrections you determine are correct. Attached is a sketch of the shaft as we see it.
