Dear Sir:

We have reviewed your letter of September 21, 1979 regarding multiple equipment failures and surveillance testing errors.

We have not yet identified an instance where an error committed during surveillance testing in conjunction with an equipment failure could result in a condition more severe than those which have been analyzed in the licensing of the Beaver Valley Power Station.

Instructions have been issued to all members of the Station Onsite Safety Committee and to all licensed operating personnel. The instructions state that each incident report and licensee event report which is prepared as a result of any incident which occurs during the operation of the Beaver Valley Power Station shall be reviewed to assure that multiple equipment failures in safety-related systems will be quickly identified, reported and analyzed to detect whether potential failure modes not previously considered could lead to a significant reduction in the ability of safety systems to function as designed.

We have also notified the individuals performing the periodic review of the Beaver Valley Power Station engineered safety features surveillance tests to determine that appropriate precautions are included to ensure that plant operators and supervisors are aware of the importance of avoiding challenges to the protection features of the facility.
