_filJBRY JINTT 2 SUE D. DUNN (804)-357-3184 Cause & Corredive At:tiun lo Prevent Recurrence trip caused by "A" S/G Lo-Lo a result of 11 B" MFP tripping The "B" MFP tripped as a result of Instrument Technicians working on the recirculation flow circuitry.

The motor protection logic circuit sensed a loss of flow and tripped the motor breaker. Repairs to the recirculatior flow switch were completed ai1d the "B' MFP was.returned to service. Reactor trip caused by "A" S/G Lo-Lo Level as a result of the "A" MFRV failing shut. The 11 A 11 MFRV failed shut due to *a breal< in InstJ;unwnt Air line supplying it. The air line was replaced.

At 1145 the primary boric acid concentration was increased to 2094 ppm in preparation for refu*ling.

At 2000 the reactor head was removed in preparation for defueling the reactor. At 0001 the defueling of the reactor connnenced.

At 0327 the last fuel assembly was removed from the reactor. At 1125 the reactor head was replaced and left unbolted.

This reporting period ends with the unit at CSD and defueled in preparation for S/G replacement.

UNIT 2 SEPTEMBER l SEPTE..'11BER 2 SEPTEMBER 4 SEPTEMBER 5 SEPTEM:BER 8 SEPTEMBER 9 SEPTEMBER 14 SEPTEMBER 20 SEPTEMBER 21 SEPTEMBER 30 e SUMP.LARY OF OPERATING EXPERIE-This reporting period begins with the unit at 70% power with a power escalation at 3%.per hour in progress.

At 1436 the reactor tripped on 11 A" S/G Lo-Lo Level as a result of 11 B" MFP tripping. "B" MFP tripped as a result of Instrument Technicians working on the lation flow circuitry.

The reactor was taken critical at 1545 and the generator synchoronized to the line at 1733. Cormnenced Power escalation.

The unit achieved 100% power at 1559. The reactor tripped at 0216 on "A" S/G Lo-Lo Level as a result of the "A" MFRV failing shut. The 11 A 11 MFRV failed shut as a result of a ken Instrument air line. The air line was repaired and the reactor was critical at 0336. The generator was synchronized to the line at 0552. Cormnenced power escalation.

At 1725 secured power increase at 79% due to sodium carryover in the main steam system. At 2240 ced power reduction to 60% in preparation for securing 11 B 11 MFP for seal repairs .. Secured power decrease at 0050 5 stopped riBu MFP and cormnenced power escalation to 70%. At 1826 "B" MFP was returned to service. At 1908 cormnenced power escalation.

Unit achieved 100% power at 0615. At 0157 reduced power to 90% for the 11ain Steam Carryover Test. At 0337 reduced power to 75% for Main Steam Carryover Test. At 0549 conducted a 10% step loss of load test and at 0551 conducted at 10% step power increase test. At 0600 commenced power escalation and reached 100% power at 0730. At 1155 reduced power to 90% in preparation for changing Tave Program. After completion of change to Tave Program, a power escalation began and power reached 100% at 1751. At 1828 reduced power to 95% for Main Steam Carryover Test. At 2039, after the Main Steam Carryover Test was completed, increased power to 100%. This reporting period ends with the unit at 100% power. LITY LICENSE OR TECHNICAL SEPTEMBER, 1980 The Nuclea-:-Regulatory Co::!llllission issued, on September 5, 1980, Amendment No. 61 to the Operating Licenses for the Surry Power Station Units No. 1 and No. 2. The amen6ments result fron the NRC Staff's review *and approval of the Surry guards Contingency Plan. Of significance, paragraph 3.J of Operating Licenses D?R-32 and DPR-37 has been added to read as follows: * "-The licensee shall fully implement and maintain in effect all provisions of the Co;:::iission-approved Safeguards Contingency Plan, including amendments and changes made pursuant to the authority of 10 CFR 50. 54 (p). The approved Contingency Plan consists of documents withheld from public.di?closure pursuant to 10 CFR 2. 790(d) identified as "Surry Power Station Safeguards Contingency Plan" dated May 1, 1980 submitted pursuant to 10 CFR 73.40. The Contingency Plan shall be fully implemented, in accordance

The Nuclear Regulatory Commission issued, on May 23, 1980, a Memorandum and Order requiring that all safety related electrical equipment in operating reactors be fully envirorunental qualified by June 30, 1982. To meet this objective, the Nuclear Regulatory Commission issued, on August 29, 1980, an Order for Surry Power Station Units No. 1 and 1lo. 2 requiring the submittal of all information to support a safety evaluation of safety related electrical equipment by November 1, 1980. ** ,.,_

There were n,o reported instances of significant adverse env'iroilmental impact. On September 1, 1980 the temperature change at the station discharge exceeded 3°F per hour due to a Unit 2 r_eactor trip.* On September 4, 1980 the temperature change at the station discharge exceeded 3°F per hour on two occassions.

The first event was due to a Unit 2 reactor trip and the second event to a rapid rampdown in power to remove the "2B" MFP from service. These events were allowable in accordance with Technical 4.14. .-.* FUEL HAHDLIHG UNIT 1 SEPTDIBER, 1980 There were no fuel shipments during this reporting period. The entire core from the reactor was moved to the Spent Fuel Pool. DURE REVISIONS THAT CHA.NGED T TING MODE DESCRIBED IN THE FS , SEPTEMBER, 1980 None during this reporting period.

Unit 1 Inservice Inspection is in progress and is being conducted by Westinghouse in accordance with VEPCO Surry Technical Specifications.

To date the inspection has been satisfactory.

2. Conducted a Magnetic Particle Inspection of all three main feed water piping tration welds. No indications were noted. 3. Completed an Ultrasonic Inspection of 10 low point welds in the Recirculation Spray System and 10 low point welds in the Containment Spray System with no reportable indications noted. 4. Commenced a Radiographic and Ultrasonic Inspection of the main feedwater 16 11 x 14" reducer welds and radiography on the 3" auxiliary feedwater tie-in lines. I '
