This LER documented thc first condition identified under thc Tcchnical Specification Surveillance Review Program in response to Generic Letter (GL)96-01,"Testing of Safety-Rclatcd Logic Circuits." By letter L-96-147, dated Junc 18, 1996, FPL submitted LER 250/96-004-02, Identification of Inadequate Surveillance Testing, to document the second condition under the Technical Specification Surveillance Review Program in response to GL 96-01, regarding inadequate surveillance of thc 4KV D Bus clearing function.The attached revised LER, 250/96-004-03, documents thc third condition idcntificd under this continuing program, regarding inadequate surveillance procedures for thc main steam linc isolation function.The revised LER, 250/96-04-3, is being provided in accordance with 10 CFR 50.73(a)(2)(v).

FPL will report any future cvcnt identified as a result of thc GL 96-01 review as a supplement to LER 250/96-004.
