NOTICE OF VIOLATION Illinois Power Company Docket No. 50-461 As a result of the inspection conducted on June 2 through July 9,1986, and in accordance with 10 CFR Part 2, Appendix C - General Statement of Policy and Procedure for NRC Enforcement Actions (1985), the following violations were identified:

1. American National Standard (ANSI) N45.2.3-1973, as implemented by Regulatory Guide 1.39, Revision 2, and Clinton Power Station Final Safety Analysis Report Chapter 1.8, requires that areas shall be kept sufficiently clean and orderly that construction activity can proceed in an efficient manner that will produce and maintain quality in conformance with specified requirements.

Additionally, Clinton Power Station procedure 1019.01, Section 8.2.1.1 requires that where accumulations of material occur, such as oily rags, the material shall be promptly removed.

Contrary to the above, on June 16, 1986, a large diameter test hose in the Fuel Building was not routed in an orderly manner, in that a small diameter instrument sensing line was used as a support for the test hose, potentially damaging the sensing line. Accumulated materials, such as oily rags, were not promptly removed from around both the Division I and II safety related emergency diesel generators, and test equipment was found stored inside an energized diesel generator control panel.

Additionally, the upper covers on an instrumentation panel were left open with no protection, and no deficiency had been written. All of these situations represent conditions which hinder maintaining quality in conformance with requirements (461/86041-02).

This is a Severity Level IV violation (Supplement II).

2. 10 CFR 50, Appendix B, Criterion V, as implemented by the Illinois Power Company Quality Assurance Manual Chapter 5, requires written procedures, instructions, and drawings to be daveloped and used for activities affecting quality.

Contrary to the above, on June 18, 1986, two Deficiency Identification tags associated with completed and closed Maintenance Work Requests on a safety-related diesel generator were found not removed as required by CPS procedure 1029.01, " Preparation and Routing of Maintenance Work Request" (461/86041-03). This condition represents a repeat failure to follow CPS 1029.01 as documented in Inspection Report No. 50-461/86023(DRS).

3. 10 CFR 50, Appendix B, Criterion XI, as implemented by the Illinois Power Company Quality Assurance Manual Chapter 11, requires the control of a test program which will assure that the safety-related structures, systems, or components being tested meet specified performance criteria.

Contrary to the above, on June 18, 1986, the Local Leak Rate Test performed by plant staff was not adequately controlled or scheduled to assure that no interference with the Air Balancing Test of the safety-related High Pressure Core Spray System room existed such that both tests would meet the specified performance criteria (461/86041-04).

This is a Severity Level V violation (Supplement II).

The inspection showed that action had been taken to correct the identified violations and to prevent recurrence. Consequently, no replies to the violations are required, and we have no further questions regarding this matter.
