This is in response to IE Inspection Report Nos. 50-556/Rpt. 79-04 and 50-557/Rpt.

79-04 requesting Public Service Company's Corrective Action for tb deviation identified during the October 2-5, 1979 NRC inspection. The inspection revealed that an unapproved. form, " Preventative Maintenance Record", was substituted in place of Form B-3038 for a 16 inch gate valve (MPL E 22F001) stored in Warehouse C. Form B-3038 (8-78) titled " Equipment Storage Maintenance Card" described and approved for use in Procedure FPPM 4.8, " Storage and Preservation of Material and Equipment". Revision .1, has been revised. The revised form (Form B-3038 Rev.1,10-79) was incorporated into FPPM Interim Instruction Letter No. 4.8 dated October 30, 1979, and approved for use. FPPM Procedure 4.8, Revision 1 is currently being revised to include the revised form and will be issued for use in Revision 2 by December 15, 1979. All " Equipment Storage Maintenance Cards" presently in use have been replaced with the revised " Preventative Maintenance Record". Appropriate personnel were instructed in the use of the revised check-list and were counseled on the need for compliance with written procedures.

This refers to the inspection conducted by Mr. W. C. ilubacek of our staff on October 2-5, 1979, of the implementation of your quality assurance pro-gram for construction activities related to your application for a con- ,

struction permit for Black Fox Station, Units No.1 and 2, and to the discussion of our findings with Mr. T. N. Ewing and members of your staff at the conclusion of the inspection.

Areas examined during the inspection and our findings are discussed in the enclosed inspection report. Within these areas, the inspection consisted of selective examination of procedures, representative records, interviews with personnel, and observations by the inspector.
