removed nor the valves repositioned.

for the Ta9ging System (AP-15} is being revised to require a log entry by the Senior Shift Supervisor

when a tagging request is not released upon sign-off by the supervisor

Heatup of the unit began on July 7, 1980 and Mode 4 established.

The unit entered Mode 3 *(Hot Standby) on July 9. Cooldown to Mode 4 was initiated

when it was discovered

The LER investigations

revealed that these were isolated dents. We feel the corrective

action conunitted

to in the ated LERs is sufficient

to prevent recurrence

and no further action is anticipated.
