10CFR21 Notification for York PIN: TCM-1 A Temperature Control Modules The purpose of this letter is to provide the NRC with a report in general conformity of the requirements of 10CFR Part 21.21. On June 18, 2021 S TP Nuclear Operating Company returned a York PIN: TCM-lA temperature control module with the reported issue that the module would not respond in auto or manual vane control and that internal relays IR and 2R would not pick up.
The subject temperature control module was previously supplied by STP Nuclear Operating Company to UCI for refurbishment and modification, after which was supplied by UCI on March 13, 2017. Troubleshooting performed by UCI identified that the secondary of the Basler Electric BE13410001 transformer (component Tl) that is used to isolate and step down the input voltage from 115VAC to 22V AC had an open circuit. When the subject transformer was bypassed the module operated as designed. UCI is unable to determine the cause of the transformer failure. Post refurbishment/modification testing performed by UCI included verifying functionality of the unit before and after performance of a 48 hr. burn-in period.

Failure of the subject transformer will result in an open circuit at the input power to the temperature control module resulting in the module being inoperable.
(v)        The date on which the information of suc/1 defect or failure to comply was obtained.
UCI issued a Return Material Authorization on November 11, 2020 & the items were received at UCI for evaluation on June 18, 2021.
(i*i)    In the case of a basic component wl1ich contains a defect or fails to comply, the number and location of these components in use at, supplied for, being supplied for, or may be supplied for, manufactured or being mamifacturedfor one or more facilities or activities subject to the regulations in this Part.
The following temperature control modules were supplied by STP Nuclear Operating Company for safety-related refurbishment and/or modification by UCI and were determined as containing or are likely to contain the subject transformer.

UCI has implemented an increased energized burn-in test period from 48hr. to 100hr. to provide greater assurance that the transformers installed on the modules will not fail in the manor described here-in. Prior to and following the burn-in period the modules are subjected to functional testing.
(viii)    Any Advice related to the defect or failure to comply about the facility, activity, or basic component that has been, is being, or will be given to purchasers or licensees.
None.
(ix)      In the case of an early site permit, the entities to whom an early site permit was transferred.
