One remote shutdown instrumentation channel inoperable contrary to Technical Specification 3.3.3.5.

Conditions Prior to Occurrence:

Mode 1 power operation (98%)

Description of Occurrence:

At 0330, while performing daily Operating Surveillance (SP-300) it was determined that DH-39-TT, Decay Heat Cooler 3A outlet remote shutdown temperature indicator, was inaccurate as com-pared to the idle 3A cooler inlet temperature. Further investi-gation revealed that RTD DH-2-TE had failed. Maintenance restored indicator operability on 23 February 1979 at 0830 following RTD replacement.

Designation of Apparent Cause:

The cause of this event is attributed to a failed RTD, DH-2-TE Analysis of Occurrence:

No effect upon public health and safety as DH-2-TE provides indication only and does not affect decay heat cooling capability.

Corrective Action:

The RTD was replaced, the system was recalibrated and the indicator (DH-39-II) was returned to service. No further corrective action required.

Failure Data:

First occurrence of an event reported under this Technical Specification.
