Thisreviewwasintendedtoassesstowhatextentprocedural requirements mayhavecontributed totheeventsandwhat,ifany,procedural revisions couldbemadetopreventrecurrence.
Itshouldbenotedthattheapplicable maintenance procedure waspreparedbasedontheinstructions suppliedwiththebreakersbythebreakermanufacturer (I.T.E.instruction'No.
1B-8.2.7-2).
Discussions withtheBBCrepresentative revealedthatperiodiccleaningandlubrication oftheclosingmechanism wasnotaddressed intheapplicable BBCtechnical manualbecausethelubrication performed atthefactorywasconsidered tobepermanent andwasmeanttolastforthelifeoftheunit.Therefore, reviewconcluded thatbecausethemanufacturer's instructions contained norequirements forperiodiclubrication ofthebreakerclosingmechanism, ourmaintenance procedure similarly didnotrequirethistobedoneduringroutinemaintenance activities.

Discussions withtheBBCrepresentative revealedthatperiodiccleaningandlubrication oftheclosingmechanism wasnotaddressed intheapplicable BBCtechnical manualbecausethelubrication performed atthefactorywasconsidered tobepermanent andwasmeanttolastforthelifeoftheunit.Therefore, reviewconcluded thatbecausethemanufacturer's instructions contained norequirements forperiodiclubrication ofthebreakerclosingmechanism, ourmaintenance procedure similarly didnotrequirethistobedoneduringroutinemaintenance activities.
