This letter is to confirm the telephone conversation of December 17, Q

1985 between American Electric Power Service Corporation and Mr. W. Jones,

NRC Office of Inspection & Enforcement, regarding notification made pursuant to Title 10 CFR Part 21.

During the process of implementing plant modifications and performing the welding associated with the modifications, it became evident that a number of weld electrodes with incomplete flux coating were issued from the supply room. These welding electrodes were supplied to the D. C. Cook

Plant by Pooch Welding Supply Company of Benton Harbor, Michigan, and were manufactured by Airco, Inc. of New Jersey. Based on the Non-Destructive Examination (NDE) records and interviews with the welders, we believe that the defective weld rods were not used in the Cook Plant and that this issue did not constitute a substantial safety hazard at the Donald C. Cook Nuclear Plant. Nevertheless, notification has been made to the NRC so that other users of similar weld rods could be notified. Additional details are included in the attachment.

DONALD C. COOK NUCLEAR POWER PLANT ATTACHMENT TO 10 CFR 21:0048 LETTER Incomplete Flux on Weld Rods Background and Discovery of Defect The defective weld rods with incomplete flux were identified by a welder when some weld rods were issued from the tool crib at the Donald C.

Cook Nuclear Plant. The welder who noticed the incomplete flux returned the welding electrode to the tool crib.

Plant Quality Control (QC) and the supplier of the weld rods were notified.

Nature of the Defect Condition Report 12-08-85-1747 was initiated on September 3, 1985 when it became evident that a significant number of weld rods with incomplete flux coating was issued from the I&MECo tool crib. The incomplete flux coating is limited to E7018 weld rods, which are a low-alloy, low-hydrogen carbon steel rods.

The defective weld rods were noticed by a welder, and they were

returned to the tool crib.

Plant QC was notified of the condition and an evaluation was performed.

It was determined that this could be a manufacturing defect.

Approximately 780 lbs. of the weld rods from this heat / lot number were

.either used or discarded at the plant during the time period July 9, 1985 through October 2, 1985. Plant QC has discussed this with the welders who used the weld rods from this batch and noted that the welders discarded the weld rods whenever they noticed incomplete flux on the weld rods. About 540 lbs. of uudaed weld rods from this heat / lot number were returned to the supplier.

The AEPSC Chief Metallurgist has stated that the defective rods would produce welds containing porosity and oxidation. This condition would be detected by the NDE examination (visual) that is part of the installation procedures for safety-related applications.

Corrective Actions Taken

A Condition Report of the event was issued and the Quality Control Department at the Cook Plant examined the defective weld rods.

Interviews were conducted with I&MECo welders and Catalytic Company welders concerning

.4 the use of this weld rod. All welders interviewed indicated that they had seen this condition and that such rodo have been discarded. Moreover, the use of such defective weld electrodes would have created defective and unacceptable welding. No such unacceptable weld inspection records were identified for the welding performed during the subject time period, i.e.,

from July 7, 1985 to September 5, 1985.

The manufacturer of the defective weld rod, Airco, Inc., has visited the plant and confirmed that the batch in question was the only one in which the defective weld rods were issued.

Based ca the above evaluations there is a reasonable assurance to conclude that the above-noted defective weld electrodes were not used at Cook Plant for safety-related applications.
