2. Facility: Crystal River Unit #3 .
3. Report Date: 17 January 1978
4. Occurrence Date: 27 December 1977
5. Identification of Occurrence:

The "3B" emergency diesel gene::ator failed to start contrary to Technical Specification 3.8.1.1.b.

6. Conditions Prior to Occurrence:

Mode 1 power operation.

7. Description of Occurrence:

At 0705, while redng the Monthly Emergency Diesel Generator Surveillance I Procedura, SP-354B, "3B" emergency diesel generator failed to start for the 3 mechanalysis run. The Crystal River Units 1 & 2 Shift Supervisor was notified of entry into the 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> Action Statement of Technical Specification 3.8.1.1 and to perform the Cable Tunnel Surveillance SP-322. The Breaker Verification Surveillance SP-321 was initiated and the operability of "3A" emergency diesel l generator was verified. Maintenance Work Request 0-5874 was initiated and the "each 8 hour9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />" surveillance requirement was met until the "3B" emergency diesel was resnored operable at 1839 on 27 December 1977.

8. Designation of Apparent Cause: ,

i Maintenance revealed that foreign matter in the governor was the cause of this event. '

9. Analysis of Occurrence:

There were no safety implications to the plant or general public, as the "3A" emergency diesel generator was available and demonstrated operable.

10. Corrective Action:

No corrective action taken as cleaning of .the governor restored "3B" diesel generator operable. "3A" diesel governor was inspected and the oil for both diesels was changed. *

11. Failure Data:

This is the third occurrence of this event as reported by LER's 77-94 and 77-128.
