Item l* Infraction Technical Specification 3.0.4 states, "Entry into an OPERATIONAL MODE or other specified applicability condition shall not be made unless the conditions of the Limiting Condition for Operation are met without reliance on provisions contained in the ACTION state-ments unless otherwise excepted."
U.S. Nuclear Regulatory  
Contrary to the above, the unit was heated up to Mode 3 on July 9, 1980 with the Containment Spray System and the steam driven Auxiliary Feedwater Pump inoperable. On July 14, 1980, valves 21CS6 and 22CS6 were found tagged closed, rendering the Containment Spray System inoperable (Technical Specifications 3.6.2.1). On July 13, 1980 valves 21MS45 and 23MS45 were found tagged closed, rendering the steam driven Auxiliary Feedwater Pump inoperable (Technical Specifications 3.7.1.2).                         In each case, the tagging predated the entry into Mode 3.
