APPENDIX A NOTICE OF VIOLATION i

As a result of the inspection conducted on May 20 - June 25,1982, and in i dCCordance with the NRC Enforcement Policy, 47 FR 9987 (March 9, 1982), the following violation was identified.  !

. t Technical Specification 6.3.D.1 requires that each radiation area in which-the intensity of radiation is greater than 100 mrem /hr shall be barricaded i and posted as a high radiation area.

Contrary to the above, the requirement that each high radiation area be' i barricaded and posted as a high radiation area was not met in that on f 4

June 3, 1982, the intensity of radiation was 120 mrem / hour and 200 mrem / hour at the edge of the barricades around two high radiation areas in the torus  ;

area of the Unit 2 Reactor Building.

I This is a Severity Level V Violation (Supplement IV.E) and is applicable to '

Unit 2.

Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or '

explanation in reply, including: (1) admission or denial of the alleged viola-

• tion; (2) the reasons for the violation if admitted; (3) the corrective steps

{ which have been taken and the results achieved; (4) corrective steps which will '

t be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
