APPENDIX A NOTICE OF VIOLATION i

l As a result of the inspection conducted on May 20 - June 25,1982, and in i
dCCordance with the NRC Enforcement Policy, 47 FR 9987 (March 9, 1982), the following violation was identified.
t Technical Specification 6.3.D.1 requires that each radiation area in which-the intensity of radiation is greater than 100 mrem /hr shall be barricaded i
and posted as a high radiation area.
Contrary to the above, the requirement that each high radiation area be' i
barricaded and posted as a high radiation area was not met in that on f
June 3, 1982, the intensity of radiation was 120 mrem / hour and 200 mrem / hour 4
at the edge of the barricades around two high radiation areas in the torus area of the Unit 2 Reactor Building.
I This is a Severity Level V Violation (Supplement IV.E) and is applicable to Unit 2.
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) admission or denial of the alleged viola-tion; (2) the reasons for the violation if admitted; (3) the corrective steps

which have been taken and the results achieved; (4) corrective steps which will t
be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
