MAR 4 1987 ENCLOSURE 1 NOTICE OF VIOLATION Tennessee Valley Authority Docket Nos. 50-259, 50-260, and 50-296 Browns Ferry License Nos. DPR-33, DPR-52, and DPR-68 During the Nuclear Regulatory Commission (NRC) inspection conducted on November 1

- December 31, 1986, a violation of NRC requirements was identified. The violation involved failure to correct an identified condition adverse to quality.

In accordance with the " General Statement of Policy and Procedure for NRC Enforce-ment Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed below:

10 CFR 50 Appendix B, Criterion XVI requires that measures to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material and equipment, and nonconformances are promptly identified and corrected.

Contrary to the above, a condition adverse to quality was identified and documented on Maintenance Request (MR) A-718016 dated October 15, 1986; however, the identified deficiencies were not corrected prior to closing out the MR on October 24, 1986 with the disposition of "all work / testing complete." The MR described that the flexible conduits installed from the junction boxes to the valve operators for the Main Steam Isolation Valves (MSIVs) had come apart and requested that the conduits be repaired or replaced and that the internals of the junction boxes and limit switches be inspected for corrosion. Valves 2-FCV-1-15, 27, and 38 were specifically identified. An inspection on November 10, 1986 identified that conduit on valves 2-FCV-1-27 and 38 were still deficient.

This is a Severity Level IV Violation (Supplement I) and is applicable to Unit 2 only.

Pursuant to the provisions of 10 CFR 2.201, Tennessee Valley Authority is hereby required to submit to this Office within 30 days of the letter transmitting this Notice a written statement or explanation in reply including: (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the

/ corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
