ENCLO3URE 1 NOTICE OF VIOLATION Tennessee Valley Authority Docket No. 50-260 Browns Ferry Unit 2 License No. DPR-52 During the Nuclear Regulatory Comission (NRC) inspection conducted on February 9-13 and 17-20,1987, a violation of NRC requirements was identified.

The violation involved failure to follow instruction PMI-8.2 in that the i

assistant plant manager performed the technical review of Engineering Change Notice (ECN) P-0859 rather than the cognizant system engineer and the Unit 2 shift engineer's ECN files did not contain required documentation pertinent to ECN P-0859.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2 Appendix C (1986), the violation is listed below:

10 CFR 50, Appendix B, Criterion V, and the accepted Quality Assurance (QA)

Program (TVA-TR75-1A, Revishn9) collectively require that activities affecting quality be prescribed by procedures and be accomplished in accordance with these procedures.

A.

Plant Manager Instruction (PMI) 8.2 Plant Design Change Review, Revision 0 (July 9-21, 1986), paragraph 4.2, states that the cognizant system engineer will complete a review of the Engineering Change Notice (ECN) package for technical. accuracy, operability considerations, Technical Specification impact installation feasibility, restrictions and document the completion of this review on ECN Review Form PMI-2.

Contrary to the above, on July 15, 1986, the Assistant Plant Manager signed for the review of ECN P-0859.

B.

PMI-8.2, Plant Design Change Review, Revision 1 (October 24,1986 to present), paragraph 4.4 states that the shift engineer will maintain a file containing the signed PMI-2 and PMI-3 forms and any other pertinent ECN information deemed necessary.

It also states that the shift engineer is responsible for documenting the app)roval to work the specific work plans and Inspection Reports (WP & irs associated with an approved ECN on a WR & 1R Work in Progress Form (PMI-3).

Contrary to the above, an inspection of the Unit 2 Shift Engineer's ECN files on February 11, 1986, indicated that these files did not contain the ECN approval forms PMI-2 and PMI-3, the unreviewed safety question determination form work, and the ECN package P-0859.

This is a Severity Level V violation (Supplement II).

Docket No. 50-260 Browns Ferry Unit 2 License No. DPR-52 Pursuant to the provisions of 10 CFR 2.201, Tennessee Valley Authority is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including (for each violation):

(1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending the response time.
