NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-374 As a result of the inspection conducted on December 30, 1986 through January 27, 1987, and in accordance with 10 CFR Part 2, Appendix C -

General Statement of Policy and Procedure for NRC Enforcement Actions (1985), the following violation was identified:

10 CFR 50, Appendix B, Criteria VI, " Document Control" states in part, "These measures shall assure that documents, including changes, are reviewed for adequacy and approved for release by authorized personnel ..... ."

10 CFR 50 Appendix B, Criteria XIV, " Inspection, Test, and Operating Status" requires measures to be established for indicating the operating status of structures, systems and components of the nuclear power plant.

Contrary to the above The following examples of inadequate procedures and loss of system status were identified:

a. On January 6, 19S7, the Unit 2 125V battery 2B Service Test Discharge Procedure LTS-700-7 was found to be inadequate. The test procedure was inadequate in that it did not require Unit 2 Division II 125V batteries to be declared inoperable and entered in the Degraded Equipment Log (DEL) when the test put the batteries in an inoperable condition due to the discharge. This condition contributed to a loss of equipment status by the operations personnel as follows: The Unit 2 Division II 125V batteries had been discharged by the test and were in the process of being recharged. In this condition, the system could not meet the Technical Specifications for battery surveillance requirements and should have been declared in operable. Operations personnel were not fully aware of the status of the batteries which had not been declared inoperable, and uncrosstied the crosstie between the Unit 1 and Unit 2 Division II 125V batteries. With the batteries uncrosstied, the Unit 2 Division II 125 VDC distribution system was inoperable and the licensee

, was not aware this condition existed.

b. On January 6,1987, the procedure for " Battery Equalizing Charges,"

LOP-DC-07, was found to be inadequate. An Equipment Operator (E0) responsible for recharging the Unit 2 Division II 125V batteries was using LOP-DC-07 and some supplemental handwritten instructions to help clarify the battery charger amperage requirements. The procedure did not state or reference the Technical Specification category B requirement for operability of the batteries and the handwritten instruction misled the operator resulting in Unit 1/ Unit 2 Division II 125V batteries being uncrosstied prior to the Unit 2 Division II 125V batteries being operable.

This is a Severity Level IV violation (Supplement I).

Notice of Violation 2 Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.
