lI NOTICE OF VIOLATION The Cleveland Electric Illuminating Docket No. 50-440 Company As a result of the inspection conducted on March 4 through April 8, 1986, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violations were identified:

10 CFR 50, Appendix B, Criterion XIV, as implemented by Cleveland Electric Illuminating Company's PNPP Quality Assurance Plan, Section 14, General Requirement 14.1, required that procedures for controlling inspection, test and operating status address the requirements for identifying the operating status, and require records to be maintained which reflect the operating status of equipment and systems.
Contrary to the above, Prior to March 7, 1986, procedures for controlling inspection, test and operating status following preoperational testing did not address all instrument valve position requirements necessary for identifying the operating status of plant instruments nor did they require that records of instrument valve positions be maintained as necessary to reflect the operating status of plant instruments.
This is a Severity Level IV Violation (Supplement I).

10 CFR 50, Appendix B, Criterion V, as implemented by Cleveland Electric Illuminating Company's PNPP Quality Assurance Plan, Section 5, General Requirement 5.1.6, required that personnel shall perform their tasks in accordance with current approved procedures and instructions.
Perry Administrative Procedure (PAP)-1401, Revision 1, " Equipment Tagging,"
required that tag-out sheets be reviewed by the person in charge, reviewer, and Unit Supervisor, to assure that the tag-out sheets appropriately identified all necessary control and protection.
Contrary to the above, On March 3, 1986, Work Order (WO) No. 86-0735 was issued to the field with tag-out sheets that were inadequately reviewed, in that, the tag-out sheets did not identify the need for the upper fuel pool level to be below the elevation of the siphon-breaker associated with the spectacle flange that was to be rotated.
This is a Severity Level V Violation (Supplement I).
