NOTICE OF VIOLATION Gulf States Utilities Docket: 50-458/86-16 River Bend Station License: NPF-47 During an NRC inspection conducted on March 31 through April 4,1986, three violations of NRC requirements were identified. The violations involved failure to follow procedures (two instances) and failure to have an adequate procedure.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violations are listed below:

A. Failure to Have Adequate Procedures Technical Specification 6.8.1 required written procedures including those of Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Appendix A of Regulatory Guide 1.33 specified procedures for control of Measuring and Test Equipment. These procedures were required to be a type appropriate to ensure that tools and other measuring and testing devices were properly controlled, calibrated, and . adjusted at specified periods to maintain accuracy. ADM-0029, Revision 6, dated January 21, 1986, implemented this requirement. ,

1i Contrary to the above, in a memo dated March 25, 1986, the licensee

' removed torque wrenches from the controls of ADM-0029 and established a new method of controlling torque wrenches without having implemented a written, approved procedure.

'. This is a Severity Level IV violation (Supplement I) (50-458/8616-01). .

B. Failure to Follow Procedures

, Technical Specification 6.8.1 required written procedures including those of Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Appendix A of Regulatory Guide 1.33 specified required administrative procedures including one for procedural adherence. ADM-0005, Revision 4, dated February 17, 1986, provided the procedure for Station Document Control and requires that document revisions be incorporated into station manuals within 10 working days for stations located off site.

Contrary to the above,17 document transmittals were found at the Emergency Operations Facility that were older than 10 working days and not incorporated into station manuals.

This is a Level V violation (Supplement I)(50-458/8616-03).

2 C. Failure to Follow Procedures Technical Specification 6.8.1 required written procedures including those of Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Appendix A of Regulatory Guide 1.33 specified required administrative procedures including one for procedural adherance. SSP-1-004, Revision 0, dated February 18, 1986, provided the procedure for Station Document Control and states that report IS-217 is the primary document for identifying outstanding changes against documents.

Contrary to the above, a modified 15-217 report was being used to identify outstanding changes for BZ drawings. The modified report was not addressed by SSP-1-004.

This is a Level V violation (Supplement I) (50-458/8616-02).

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities Company is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violations if admitted,

2) the corrective steps which have been taken and the results achieved,
3) the corrective steps which will be taken to avoid further violations, and
4) the date when full compliance will be given to extending the response time.
