Warren, Resident Inspector NOTE: Personnel noted above were present at exit meeting on July 2, 198 .0 General Information Licensees have discovered Limitorque Motor Valve operators with jumper j wires different from those tested by Limitorque in its environmental e qualification program and internal wiring of its Limitorque operators that I, either could not be identified or qualification that could not be estab-lishe .0 Applicability of Information Notice In response to the NRC Information Notice No. 86-03, " Potential Defici-encies In Environmental Qualification of Limitorque Motor Valve Operator Wiring", the licensee had reviewed the document's applicability to the Shoreham site and determined that, except for possibly six motor valve operators shipped prior to 1972, the subject Information Notice did not appear to impact environmental qualification of Shoreham's Limitorque Motor Valve Operators. This was because Shoreham's Motor Valve Operators were manufactured after 1973. Licensee memo to file No. NSD-86-791, dated April 3, 1986 indicated that the motor valve operators manufactured from 1973 to 1978 were provided with qualified wiring and that qualification testing for both Raychem Flametrol and Rockbestos SIS wire was performed by each wire manufacturer and is documented in LILCO EQDP's 88V-01, -02,

-03 and -04 and in Wyle Assessment Report No. 17464-710 _

3 The memo also indicated that any wiring changes or additions that may have been performed at Shoreham by LILCO would be acceptable since all control and instrumentation wiring delivered to Shoreham is qualified for use throughout the reactor building and documented in LILC0 EQDP's 129-02, 130-01, 159-03, 159-04, 159-08 and 484-0 The licensee concluded that the subject Information Notice was conditionally closed. The final closeout was to occur after completion of inspection of the six motor valve operators supplied to Shoreham in 1971 and random inspection of an additional six motor valve operators manu-factured 1973 and later that are presently installed at Shoreha .0 Physical Inspection The licensees I&C group performed an inspection on March 31, 1986 to address the concerns of the Information Notice No. 86-03 regarding the qualification of Limitorque Motor Valve Operator Internal wiring in accord-ance with licensee memo No. NSD-86-76 The licensee first inspected the six motor valve operators shipped to Shoreham in 1971 and then the six additional units shipped to Shoreham after 1973. Of the six units shipped in 1971, two (1831* MOV 031 A&B) contained quest;onable, unidentifiable wire jumpers from the limit switch to the torque switch and the wire jumper from switch terminal LS4 to LSS. Thue wires were replaced with qualified wiring. Limitorque units 1831* MOV 032 A&E, were not inspected because the actuators for these units had beer. repla.ed under licensee work order No. FD0P--85-006. Limitorque units 1Z91* MOV 033, 034 could not be inspected since the licensee had no records to indicate these units were back on site after having been returned to the vendo ;

Of the six post-1973 Limitorque Motor Valve Operators randomly selected i from presently installed Motor Valve Operators, all were found to have '

questionable, unidentifiable and potentially unqualified wire jumper (black) from limit switch terminal LS4 to LSS. The licensee determined that it was unlikely that the wiring was unqualified since Limitorque had designed control circuits to meet customer requirements, prepared all electrical wiring diagrams and installed the control circuit wiring prior to shipment of the motor valve operators to the valve manufacturer. A

l drawing verification performed by an NRC inspector of the D.C. Elementary Diagram No. ESK-11E1101 for Limitorque Motor Valve Operator No. 1E11 * MOV l 048 noted that the subject jumper was included on the drawing indicating i installation by Limitorque. Although the Information Notice 86-03 indicates i

that limitorque can provide or reference documentation to support qualifi-cations of wires it installed, the licensee was , unable to support or establish the qualified life of the black jumper wire from limit switch terminal LS4 to LS5 based on site records.

Limitorque units inspected by licensee. and found to contain questionable, unidentifiable wiring include:

The inspector noted that the units listed above and inspected by the licensee contained valves procured from the same valve supplier. The EQ list furnished by the licensee indicates there are 292 Limitorques with eight different valve suppliers. To date no inspections of other valve suppliers were made to determine the extent of the wiring proble The NRC inspector accompanied by licensee personnel examined Limitorque units numbered 1E41*MOV 031 A&B in the primary containment verifying licensee replacement of the questionable, unidentifiable wire jumper from switch terminal LS4 & LSS. Whik in the containment the inspector exam-ined three additional Limitorga units. Two of these units (1E41* M0V 042/1E11*MOV 048) were also found to have questionable, unicentifiable wire jumpers from limit switch term #nal LS4 to LS One of the units (1E11*MOV 048) was from the second largest valve supplier of Shoreham's Limitorque Motor Valve Operators, indicating the wiring problem was not limited to one valve supplier. The additional findings and lack of supporting documentation raised tne question regarding the qualified status of the remaining Limitorque Motor Valve Operator Discussions with the licensee regarding equipment qualifications, 10 CFR 50.49 requirements and possible return to power by Shoreham on July 7, 1986 prompted a conference call between the Region and the licensee on July 2, 198 As a result of this call the licensee agreed to do a 100% inspection of Limitorque Motor Valve Operators before returning to power and to contact the vendor (Limitorque) regarding the questionable, unidentifiable wire jumper The NRC staff, through Information Notice 86-03, considers the resolution of this issue to be part of the licensees environmental qualification program to establish and maintain the qualified status of electrical equipment within the scope of 10 CFR 50.4 CFR 50.49(J) requires a record of the qualification must be maintained in an auditable form for the entire period during which the covered item is installed in the nuclear power plant. It also require:, verification that each applicable item of electric equipment important to safety is qualified for its application, meets specified performance requirements and will perform its safety function up to the end of its qualified lif '

The license was informed that its actions with regard to the questionable, unidentifiable, wiring was a potential violation of 10 CFR 50.49 requirement (50-322/86-13-01) ,

During the containment inspection of Limitorque Motor Valve Operator internal wiring, which requires removal of the limit switch compartment cover, it was noted that Motor Valve Operator IE41*MOV 042 limit switch compartment cover had three missing bolts. In addition, after removing the installed bolts it was noted that the material for two of the bolts was aluminum instead of steel which is normally used. The licensee was not aware of any work or repair in process nor were any tags found which indicated any ongoing work to account,for the missing bolts or use of aluminum bolts. This item is unresolved pending NRC review of licensee evaluation and corrective action. (50-322/86-13-02)

5.0 Unresolved Item ,

Unresolved items are matters about which more information is needed to determine whether it is acceptable or a violation. An unresolved item is discussed in Detail, paragraph .0 Exit Meeting The inspector met with licensee representatives identified in Detail, paragraph I at the conclusion of the inspection on July 2,1986 at the plant site. The inspector summarized the inspection findings which were acknowledged by the licensee. The licensee has committed to 100% in-spection of Limitorque Motor Valve Operator internal wiring and to deter-mine the extent of Limitorque limit switch compartment cover installation bolt proble At no time during the inspection was written material provided to the licensee by the inspector.
