APPENDIX A NOTICE OF VIOLATION Long Island Lighting Company Docket No.
50-322 Shoreham Nuclear Power Station License No. NPF-19 During an inspection from January 11-15, 1988, and in accordance with the "General Statement of Policy and Procedura for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement Policy), the following violation was identified:
10 CFR 50, Appendix J, Section III.C.1 requires that for Type C tests, each valve to be tested be closed by normal operation and without any preliminary exercising.
Contrary to the above, on January 13, 1988, while preparing to leak test RCIC t';rbine exhaust check valve 1E51*08V-0021, the valve was preliminarily exercised in that the test volume was overpressurized to greater than 100 psig instead of the required 40 psig test pressure.
This occurred as a result of an inadequacy in Procedure SPF 84.654.03 Revision 4.
This is a Severity Level V violation.

Pursuant to the provisions cf 10 CFR 2.201, Long Island Lighting Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
