Three Mile Island Nuclear Station, Unit 2 (TMI-2)

Operating License No. DPR-73 Docket No. 50-320 Removal of Defueling Tools From the Reactor Vessel At approximately 7:40 a.m. on Saturday, June 21, 1986, the canister seal installation tool was removed from the reactor vessel following seal installation on defueling canister D200. A radiological survey was not performed prior to storage of the tool. The cause of this event was the apparent failure of the Task Supervisor to require a radiological survey of the tool in compliance with the standing Radiation Work Permit (RWP). During the course of inquiry into the cause of this event, it has been determined that at least one similar event may itave occurred involving removal of a defueling tool from the reactor vessel in a manner not strictly in compliance with the requirements of the applicable RWP. The event of June 21, 1986, and the results of our inquiry into the matter have been the subject of several discussions between members of our respective staffs.

As a result of the above, GPU Nuclear is taking the following corrective actions in order to preclude future events of a similar nature; Task Supervisors shall include specific RWP requirements as part of o

every Reactor Building pre-job briefing.

Department /Section Supervisors shall confirm in writing that all Task Supervisors under their control have been counseled in the proper application of RWP requirements for each task within their purview and the pre-job briefing requirement noted above.

o Task Supervisors will be counseled as to the " lessons-learned" from this event with specific emphasis on proper turnover between Task Supervisors and proper communication between Task Supervisors and Radiological Support personnel.

o The " lessons learned" from this event will be included in the training material for Radiation Worker Training for both the initial and refresher courses as part of the periodic discussion of significant events. This training is part of the General Employee Training Program.

o A synopsis of this event also will be included in the TMI Info-Update to publicize the " lessons learned" from this event for all RWP-qualified personnel.

o An inquiry is continuing, via the Incident Event Report process, to determine if similar events have occurred involving defueling tool removal from the reactor vessel in a manner not strictly in i,

compliance with the required RWP.
