CONCERN: C J Parker, Powerhouse Mechanic Instrumentation, has a concern about foreman giving a copy of the redhead test to employees with the answers before the crew took the test.

T H Robertson was assigned this concern. The Investigation / Resolution concludes this issue does not raise a safety concern. Additional concerns are noted in the previous write up.

A final interview was conducted. According to Construction Employee Relations personnel, Parker has no quality concerns but would not sign his final interview affidavit because he feels stronger action should have been taken regarding his supervisor. The action documented in the Employee Relations Concerns Action Plan is appropriate.

TECHNICAL INTERVIEV:

1/2 Pipe Expansion Loops - Turbine Building. I was installing the 1/2 piping and noticed that an expansion coII was sp'ecified by the drawing.

I asked Ken Dodd about the coII. He said Installation of the coil was unnecassary. I asked why, he mentioned something about upgrading the system and therefore the coils were not required. The drawings still specified the coils. Ken then told me to get to work and do as he said (no coils). Since the piping Installation, numerous R-2A's have been written on the 1/2 piping.

- Recently al_1,,,of the pipe has been removed and replaced with flex hose.

~ I forgot to report one other concern about quality at my first interview.

.In t;he' turbine building, when I was told to repair bent " redheads" on support base' isl ates'.' I"had two options:

1. Remove the plate and replace the studs or
2. Beat the studs back into proper plumb.

I always chose the latter. (Note: C J-Parker.must have meant hanger anchor studs, not " redheads"). l.do not remember where these support plates are located.

On the ground water sump pumps outside the turbine building, I was fitting a stand pipe with a support member. The welder I was working with could not make the weld required by the drawing. I asked Dodd what should be done. Ken asked me.If the weld could be easily seen. I replied no. Ken said forget it then, thitl eid will be under water. ~As far as I know, that weld still is I can locate this weld.

Another incident I did not talk about in the first interview, concerns socke ~ t 'weild,fl.tting on the "Levitron" system near the condensor in the turbine per Dodd's Instructions, I fa!!ad to scribe building.' On this two inch the 1/16" socket weld gap l'i~n's.' Th sy" stem;4W1'ds Wriii not' gapped. An inspector came by, saw the lack of scribe line, asked me what was going on. I referred him to Ken Dodd. They talked and nothing was done about the gap problem. The welds swere made. I can loca'tF these welds. I,.do not know who the inspector was.

I believe Dodd's attitude and his way of working his men, affected quality.

I have heard of one other problem, not allowing grout.placed in incorrectly

' located holes in the' walls to cure for a week ,, prior to recatering the~ iepaired holes. "I have 'not seen this problem. -'~I' heard from a fitter named " Buck".

To the best of my knowledge, I have supplied all relevant Information concerning the topics discussed.

Affidsvit C'J Parksr d in my present crew, I would tell my foreman if I had a problem in the field. If the problem was safety related, I'd either tell my safety proctor cnd/or my foreman. I have never reported any poor quality work.

A lot I wouldn't want to be responsible for the operation of the plant.

cf people don't want to do good work. flobody complains because they're cfraid they'll get fired.

I don't have any specific examples of others doing less than quality work.

I just know there's a lot of rework.

I have supplied all relevant information to the best of my knowledge.
