Several craftsmen felt that foreman pressure could result in a lowering of work quality.

_ L FOUNDED UNFOUNDED Q-1A REQUIRED YES X NO Q-1A NUMBER Investication Of the 190 non-supervisor craftsmen who were interviewed, only five expressed a concern that the quality of specific safety-related work was allegedly unsatisfactory because of pressure for quantity of production as opposed to quality. (Indeed, only two craftsmen expressed a concern relating to pressure allegedly impairing the quality of non-safety related work.) As described below, these concerns regarding safety-related work relate to only four separate incidents involving interpass temperature or backing rings.

Significantly, only three particular supervisors were involved. The technical issues which these five craftsmen raised, which underlie the alleged production pressure, are described below.

Resolution 1.

Individuals 33 and 106 related the same incident, involving pressure from their foreman (Individual 78) for one of them to complete welding a joint that had been cooling for at least 2-3 minutes. The welders thought this pressure made the one welder violate the interpass temperature, allegedly affecting quality.

Resolution of the technical concern is adcressed in Concern No. 1.

This resolution reflects that even if it did occur as alleged, it would have had no safety impact.

Individual 78 was previously removed from his supervisory position.

Two other welders (Individuals 36 and 70) believed that production pressure from one foreman (Individual 142) or the general foreman over him (Individual 184) caused them to violate interpass temperature. Technical resolution of these issues are also addressed in Concern No. 1.

In short, even if these incidents occurred as alleged, it would not have resulted in a condition adverse to the safety of the plant. Welder (Individual 191) felt that production pressure from his foreman (Individual 142) caused him to burn through a backing ring.

Action No addit,ional technical action is required.

Personnel action will be taken in accordance with Section VI of the main report.

Conclusion Even if the production pressure alleged by these six craftsmen did occur, it did not impair the safety of any systems in the plant. In any event, the supervisors implicated in these incidents will be removed from their supervisory positions.

Further, these actions will be made known to craft supervisors in training

sessions, where this issue will be thoroughly addressed. Such actions will clearly reinforce Duke's (ontinued commitment to quality work.
