Attached is Abnormal Occurrence Report A0-26-74 which covers the failure of the lube oil pump on emergency diesel Generator 1-1.

This failure occurred during test operation of the diesel generator and there was no effect on plant operation.

An occurrence involving a leak in the primary makeup tank (T-90) was reported on 11/4/74 and initially called Abnormal Occurrence A0-25-74. Following the plant's review of this occurrence, it van re-claasified as a nonreportable event. This change was discussed with Region III on 11/6/Th.

ABNORMAL OCCURRENCE REPORT Palisades Plant

1. Report Number: A0-Th-26, Docket 50-255
2. a. Report Date: November 13, 1974.
b. Occurrence Date: November h, 1974
3. Facility: Palisades Plant, Covert, Nuchigan.
h. Identification of Occurrence: Failure of the lube oil pump during monthly testing.

5 Conditions Prior to Occurrence: The reactor was in the hot standby condition.

6. Description of Occurrence: During the monthly test running of diesel Generator 1-1 it tripped off shortly after starting. The cause of the trip was due to lov lube oil pressure. Diesel 1-2 was started immediately and was test run to prove its operability.
7. Apparent Cause: Failure of the main shaft to the lube oil pump.
8. Analysis of Occurrence: Since the failure occurred during test operation, there was no affect on other operating systems. The reason for the failure is unknown at this time.

9 Corrective Action:

a. Replaced failed oil pump wiin spare unit.
b. Failed oil pump will be rebuilt or replaced.
c. The failed oil pump shaft will be sent out for analysis to determine why it failed. Further action on Diesel 1-1 oil pump will avait the results of the analysis.
10. Failure Data:
a. There is no record of any previous shaft failures or malfunctions of this equipment.
b. Emergency diesel Generator 1-1, 3125 kVA, .8 PF, air start, water cooled. Manufacturer is Alco. Component that failed is the lube oil pump.
