m DOCKET 50-255, LICENSE DPR-20 PALISADES PLANT On August 28, 1974, we transmitted a final report regarding an abnormal occur-rence (Th-11) at the Palisades Plant. This abnormal occurrence involved a loss of d-c control power to Bus 1D feeder breaker, Breaker 72-203. The Safety and Audit Review Board subsequently reviewed this report and concluded that there were some related Ufects which vere not obvious that should have been explicitly described in the report. The purpose of this letter is to provide that explicit description.
Item 8, Analysis of Occurrence, is revised as follows to explicitly delineate the related affects:
The loss of d-c control power to Bus 1D feeder breaker results in the breakers not being able to operate in case of equipment malfunction. With no alarm resulting from loss of control power, the operator may not be aware of the i
situation.
In addition, high-pressure safety injection pumps P66A and P66C, LP safety ejection pump P67A and containment spray pump P5hA vould not have actuated automatically if required during this loss of d-c control power.
The redundant counterparts of this equipment, which are powered from different buses, were available for automatic actuation. The design of the plant is such that the remaining components capable of automatic actuation would be sufficient in the event of a Loss of Coolant Accident. Not withstanding the above, the plant was in a cold shutdown condition during this occurrence.
