This is in res ponse to the Item of Violation cited in Inspect ion No.
50-412/83-08 and listed in Appendix A (Not ice of Violation) attached to your letter to Mr. E. J. Woolever, dated July 25, 1983.
NOTICE OF VIOLATION:
10CFR50, Appendix B, Criterion XV requires that measures for control of nonconforming materials or components shall include procedures for their ident ification, documentation and disposition and shall be conduc t ed in accordance with those procedures.
The Beaver Valley Unit 2 PSAR, Sect ion 17.1. lA s t ates that the quality assurance program will conform to 10C5'R50, Appendix B.

Sec t io n 16 .0 , Rev. 3 requires that inspect ion personnel shall prepare a Nonconformance are Disposition (N&D) report fo r nonconfo rmi ng items, shall at t ach a numbered "ho ld tag" to the item, rec o rd the "ho ld t ag" number on the Master Tag Log, reference the number on the N&D report and destroy the tag when the N6D report is closed.
Contrary to the above , on June 25, 1983, the ins pector observed that an unnumbered hold tag had been attached to pipe spool SIS-69-4 A, had been recorded on the N&D report as " temporary" and had not been removed and destroyed when the N&D report was closed.

It was established that the Weld Data Sheet and In-Proces s Check List, required by that program, did not provide fo r the recording of an unsat is facto ry condit ion on an immediate ba s i s . The Dif/SQC Ins pect ion Report , however, provides fo r the pr ompt recording of s uch da t a . Because of this dif ficulty, the pr act ic e of attaching an unnumbered Hold Tag to the item, during an inves tigation of the condition, became routine.
Corrective Action In order to assure that the condition described does not recur, the following act ion has be en ini t iat ed . Schneider Power Corporation h as already been assigned a special block of numbers ap plicab le to both Nonconformance and Disposition Report s (N&D's) and Cons truc-tion Deficiency Reports (CDR's) for their use rela t ive to the ASME III Program. This block of numbe rs wilt be further broken down into blocks fo r speci fic areas ard act ivi t ies within the pr ogr am.
The use of unnumbered Hold Tags will be discontinued.
Thc Leci Ins pecto r of each ap plicab le area or act ivity will be r es pons ib ic fo r msint aining a log of those N&D a nd CDR numbe rs assigned to him. In the event that a QC Inspector finds a noncon-forming condition when the Weld Data Sheet or In-Process Check List I is ut ilized in lieu of the DLC/SQC Ins pect ion Report , the QC Inspector will af fix a nunbered Hold Tag to the item involved and obtain the appropriate N&D or CDR number from the previously refer-enced log. This nunber will then be ent e red on the Hold Tag arvi the N&D or CDR.
The N&D or CDR will then be pr oces sed in ac cordance with the existing procedures as described in the Schneider Power Corporation ASME III Manual and DLC/SQC Procedures.
In addition, a field review will be performed to correct the condi-tions of any unnumbered ASME III Hold Tags existing in the field at this time.
The above program will be implemented prior to September 1,1983.
