Fort St. Vrain i

The reactor has experienced power distribution problems due to insertion of the reserve shutdown system (boron carbide balls) into Region 27 of the graphite core. The reactor continues to operate at 70% power level a: no Tech Spec limits or procedures are being violated.

The licensee is evaluating corrective measures.

Vircil C. Surner, Unit 1 An operating license was issued August 6,1982.

The license includes a condition requiring Comission approval prior to operation at power levels in excess of 5% of full power.

Fuel load is expected to begin August 9,1982.

'he ASLB was informed on Aucust 9 that a welder knew of defective cadwelds 1 the containment and was requested to stay its initial decision issued August 4, 19E2. The Board will hold a conference call August 12 to pemit the parties to study the welder's affadavit.

This matter has received sig-nificant media attention in cortjunction with Chairman Palladino's visit to the facility on August 9,1982.

Seabrook On August 10, a fireworks storage facility located just outside the Seabrook site boundary blew up, scattering debris over a portion of the sita and causing an access road to be temporarily closed. There was one fatality and four injuries at the fireworks facility.

No injuries occurred among Seabrook employees.

San Oncfre, Unit 1 A case study report on the loss of salt water cooling event was issued. The event occurred while the plant was operating at 100% power and involved an unlikely triple failure which resulted in exceeding the plant's limiting conditions for operation. The report highlights AEOD concerns relating to simple failure vulnerability of the salt water cooling system, contamination of plant air systems, in-service testing programs, and plant technical specifications.

CRBR Fuel Cycle Environmental Review The ASLB decided to split the CRBR LWA hearings into two portions. The first portion, starting August 24 at Oak Ridge TN, will be limited to CRBRP acci-dents and effects.

The second portion will deal with fuel cycle, transpor-tation, waste management, and safeguards.

It is anticipated that the final FES supplement will be issued in late October 1982, and the second part of the hearing will be held in late 1982 or early 1983.

Fort St. Vrain - Power Distribution' Problem Public Service Companyof Colorado (PSC) identified a local power distri-f l

bution' reduction in Region 27 of the Fort St. Vrain graphite core. The i

problem was identified during routine analyses of intore instrumentation readings. It was postulated to be caused by insertion of the reserve shutdown system (boron carbide balls) into Region 27. This was confirmed 4

by detecting a zero delta-pressure across the rupture disk that retains r

the balls. The reactor continues to operate at 70% power level as no TS i

limits or procedures are being violated.

A similar event happened' during startup testing. However, removal of the balls, by vacuuming from the channel, would be considerably more dif-ficult this time because of the highly radioactive condition of the boron carbide balls.

Vircil C. Summer Nuclear Station, Unit 1 An operating license for Virgil C. Summer Nuclear Station, Unit 1 was

-l issued August 6,1982. The license includes a condition requiring Com-mission approval prior to operation at power levels in excess of 5% of full power (139 megawatts thermal). Current utility estimate for start of fuel load is Monday, August 9,1982.

Vircil C. Summer Nuclear Station, Unit 1 Intervenor Brett A. Bursey orally informed the ASLB on August 9 that a welder for Daniel Construction Company knew of 30 to 40 defective cadwelds in the containment. In a conference call on August 9 with the Board and parties, Bursey requested that the Board stay its initial decision issued August 4,1982. The Board did not rule but will hold another conference call with the parties on August 12, 1982 to permit the parties to study the welder's affidavit which will be made available by Bursey on August 10, 1982.

This matter has received significant media attention in conjunction with Chairran Pa11adino's visit to the facility on August 9,1982.

Seabrock i

At about 2:30 p.m. on August 10, 1982, a fireworks storage facility located just outside the Seabrook site boundary blew up. Debris was scattered over a nearby portion of the site causing an access road on site to be temporarily closed.

There were ho injuries to any Seabrook employees (closest persons were gate guards). Five employees of the fireworks facility were present when the explosion occurred. One is known to have been. killed.. The other four were injured and have been taken to a hospital.

Arkansas Nuclear One, Unit No.1 At 23:42 on Sunday, August 8,1982. ANO-1 "C" Reactor Coolant Pump (RCP) seal began to leak at the rate of 27 gallons per minute. The reactor was shut down and will remain shut down 3 to 4 days to replace the seal, k' hen the event occurred, the unit was starting up from a cold shutdown which was the result of a reactor trip due to a dropped control rod. The control rod drive stator had been replaced.

Effluent and Environmental (Radiolocical) Information Liouid effluents from the TMI site released to the Susquehanna River after processing, were made within the regulatory limits and in accordance with NRC requirements and City of Lancaster Agreement dated February 27, 1980.

During the period August 6,1982, through August 12, 1982, the effluents contained no detectable radioactivity at the discharge point and individual effluent sources, which originated within Unit 2, contained no detectable radioactivity.

These results will be included in a subsequent report.

No radiation above normally occurring background levels was detected in any of the samples collected from the EPA's air and gama rate networks during the period from August 4,1982, through August 12, 1982.

<6.3 E-14 < 6.3 E-14 NRC environmental TLD measurements were last reported in the TMI Program Office Weekly Status Report for the period September 27 through October 3, 1981.

In that report, it was announced that effective July 1,1981, the TLD station change frequency wobid be quarterly instead of monthly. The NRC TLD Direct Radiation Monitoring Network (environmental) results have since been reported in NUREG-0837, Volume 1, Nos.1, 2; Volume 1, No. 3 (July-September 1981); and Volume 1, No. 4 (October-December 1981).

During July through September 1981, 100 TLD's around TMI indicated gama radiation to be at the natural background levels, i.e., 0.15-0.29 mR/ day.

During October through December 1981, 85 TLD's around TMI indicated gama radiation to be between 0.1t.-0.25 mR/ day. These dose rates are consistent with natural background radiation in the TMI area.

TMI Occupational Exposure Licensee TLD (Thermoluminescent Dosimeter) records indicate the following Unit 2 occupational radiation exposures for 1982:

Thus, if each member of a population group of 1,000 people were to receive a dose of 0.001 rem (1 millirem), or if two people were to receive a dose of 0.5 rem (500 millirem) each, the total man-rem dose in each case would be one man-rem.

Licensee Radioactive Material and Radwaste Shipments On August 12, 1982, 80 drums of Unit 1 and Unit 2 contaminated laundry were shipped to Interstate Unifom Services, Inc., New Kensington, Pennsylvania.

Submerced Demineralizer System (SDS).

SDS is in a shutdown condition.

EPICOR II. The EPICOR II system is currently shetdown in a standby status.

Reactor Buildina Entries / Core Inspection.

Reactor building entries were concuttec on inurscay anc Fricay, August 12 and 13, 1982. The third closed circuit television inspection of the core was conducted during the Thursday entry. The camera was inserted into the core through the leadscrew opening in control rod 9E (the same opening which was used for the core inspection on August 4, 1932). A metal rod for probing the rubble was inserted into the core through the samb opening as the camera.

Ine results of the probe inspection are not certain because the camera operator was unable to locate the probing rod with the camera. The probe operator felt the probe contact the rubble pile and observed the probe extension rods go into the reactor vessel another 14 inches while twistirg the last extension rod. Based on perception, while handling the probe extension rods, the probe operator concluded that the top 14 inches of the rubble was relatively soft. Both the probe and camera operators were manipulating their equipment from the control rod drive platform, 40 feet above tne rubble bed.

use of the polar crane for reactor head removal in 1983. Other i

I activities conducted during the entries this week included post decontamination data acquisition below the 305 ft. elevation, component removal for accident effects studies, and general housekeeping.

l Three entries are scheduled for next week. The primary task during these entries will be the uncoupling of the nemaining control rod drive leadscrews. The proposed uncoupling will disconnect the leadscrews from the control rod spider assemblies (prerequisite for head removal). The leadscrews will be left in position inside the control rod guide tubes.

EPICOR II Prefilter Shipment. The scheduled shipment of the first EPICOR 11 prefilter was celayed by at least one week. The delay occurred after the PF-3 prefilter was placed in the shipping cask. The shipping I

procedure included steps to inert the shipping cask with nitrogen.

During the inerting, leakage was detected at the cask cover seals. The prefilter was removed from the shipping cask and licensee personnel commenced inspection of the sealing surfaces to determine the cause of the leakage. The PF-3 prefilter is the first of 49 EPICOR II prefilters scheduled for shipment off-site. The nitrogen inerting of the liners and shipping i

casks was established as a required safety precaution prior to handling and shipping the prefilters off-site.

Task Force on Medical Licensing Dr. William Walker, Section Leader of the Medical / Academic Licensing Section, NMSS attended the Annual Meeting of the American Association of Physicists in Medicine ( AAPM) in New Orleans, and briefed the Nuclear Medicine, Radiation Protection, Radiation Therapy and Government Regulations Committees on progress in Revision of 10 CFR Part 35, Human Uses of Byproduct Materials.

l The proposed medical revisions were well received and NRC was commended by Dr. Anne Wright, President of AAPM for participation in the meeting and for' our efforts to infom the professional comunity of NRC regulatory actions i

affecting it.

CRBR Fuel Cycle Environmental Review The ASLB decided to bifurcate the CRBR LWA hearings at a pre-hearing confer-ence on August 2.

The first portion of the hearing will start August 24 at Oak Ridge, TN and will be limited to contentions 1, 2, 3 and 5(b) which are related to CRBRP accidents and effects. NMSS involvement in this portion will be limited to H. Lowenberg testinony on contention 5(b)--Effects of the Avail-ability of Oak Ridge Gaseous Diffusion Plant on U.S. National Security and Energy Needs.

NMSS involvement in this project will be active in the first quarter of FY83 with preparation of responses to comments on the draft FES supplement and preparation of the final FES supplement. This will be followed by participa-tion in the second portion of the LWA proceeding in the areas of fuel cycle, transportation, waste management and safeguards.

It is anticipated that the final FES supplement will be issued in late October,1982, and the second part of the hearing will be held in late 1982 or early 1983.

i Northwestern has published a report titled " Countercurrent Steam-Water Flow in a Flat Plate Geometry." It examines a stratified flow of steam and subcooled water in a rectangular channel at inclined angles.

l Arconne National Laboratory Steady-state and transient heat transfer and fluid visualization studies have been perfomed at Purdue University. A report describing 1

the results of these tests under natural circulation conditions will be available in a couple weeks.

Data Bank l

Data from LOFT Test L2-5, which is part of the double-en'ded break series, was added to the data bank. Also, data from 100 tests of FLECHT SEASET - 21 Rod Bundle and data from the General Electric Single Heated Bundle Facility were recently received for addition to the bank.

LOFT A readiness review for the next two tests will be held August 11, 1982.

Special attention is to be placed on operator training and alternate procedures. This is deemed necessary in view of the circumstances of a test maneuver following the last test, L2-5, which led to an unexpected core voiding and heatup.

The continuing reduction in manpower on the LOFT project is makinn it increasingly difficult to maintain the target test schedule; although the next test, L6-B, is reported on schedule for August 25 to September 1. RES believes the chances of a few days slip are high.

Semiscale The nodifications to the Semiscale facility (conversion to MOD-2B) are in progress and should be completed in early November 1982.

Results of the Semiscale feed and bleed experiment (S-SR-2) are being analyzed. A letter from EG&G (North to Tiller dtd. 08/06/82, Primary Coolant System Fleed and Bleed, PM-137-82) has been received which discusses a steady-state feed and bleed analysis for both Semiscale AUG 131952 ENCLOSURE E I

l and a typical PWR. The conclusion reached is that the Semiscale experiment does not point out the existence of a definite problem regarding primary feed and bleed, but they do tend to support a concern about the relative tenuousness of the process. Transient analysis of the feed and bleed process have an important bearing on the feed and bleed process and these are currently in progress for both the Semiscale experiment and a typical PWR.

RELAP5 The interlm version of RELAP5, i.e., RELAP5/ MOD 1.5 was internally released on August 4,1982. This particular version of RELAPS has blowdown, refill and reflood analytical capability. It has gone through extensive developmental assessment and is ready for various applications by NRC and it's contractors. New and improved features of RELAP5/ MOD 1.5 include nonequilibrium heat transfer, adiabatic mass transfer, reflood modeling, jet mixer, improved separator, boron feedback, added control components (such as time delay, maximum and minimum functions), improved interphase drag, improved stratification model, steady state, interactive link, improved renodalization, and loop elevatit. checker. Internal draft reports on these new features will tw completed by the end of FY 1982.

j BWR. FIST The vessel and piping installation have been completed. The bundle has been assembled and hot checkout is schedule within a few weeks.

Control room wiring and 40-50 percent of the instrumentation wiring has been completed.

The BWR TRAC calculations of a BWR LOCA and a FIST test are nearly complete. GE feels that these calculations will serve the intended purpose of providing input to the FIST scaling study. Trouble was encountered, however, with the level tracking in the calculations.

INEL staff will repeat these calculations in an attempt both to 4

improve the scaling comparison and to impr ve the performance of the experimental level tracking model in TRAC.

FLECHT-SEASET The shakedown testing of the 163 Rod Blocked Tes.t is proceeding.

Six of the 15 shakedown tests have been completed. The expected date to complete the shakedown testing is.on August 15, 1982.

Valid reflood data for the 21 Rod Bundle Tests has been sent to INEL for input to the NRC/DAE Data Bank.

was issued (AEOD/C204). The event occurred while the plant was operating at 100% power and involved an unlikely triple failure which resulted in exceeding the plant's limiting conditions for operation. The event was later determined to j

be an abnormal occurrence. Recently (May 13,1982) maintenance operations being performed on one salt water pump while the plant was in cold shutdown caused i

a flooding of the pump bay which resulted in an additional complete loss of the salt j

water cooling system. The report highlights AEOD concerns relating to single

f ailure vulnerability of the salt water cooling system, contamination of plant air j

systems, inservice testing programs, and plant technical specifications.

The identified leakage rate was less than Technical Specification maximum allowable, but had recently increased; however, it still remained below allowable.

I The licensee has about 2 more days remaining to complete low power physics i

testing and expects the shutdown and repairs to result in a 6 to 8 day delay 4

in the completion of low power physics testing, natural circulation testing j

and readiness for power levels greater than 5% (mode 1). This delay will result in a projected date for 5% power of about August 29, 1982.
