SUMMARY
OF WEEKLY INFORMATION REPORT Week Ending December 11, 1981 Diablo Canyon PG&E has filed a response dated December 4 to the Comission Memorandum and Order of November 19, 1981, that describes the technical qualifications and independence of the companies it proposes to use for the design reverification program, a program plan for the studies, and an overall management plan for conducting the studies. TMI-2 Cleanup Met Ed has established a Technology Assessment and Advisory Group which will provide independent technical assessment and advice on proposed decontamination and defueling activities at TMI-2. Funding support will be provided by DOE. NFS - West Valley The U. S. Court of Appeals for the Second Circuit has reversed and remanded the decision of the U. S. District Court for Western New York regarding possession and control of the West Valley site. It appears that NFS may remain in control of the West Valley site for some still undefined period. Emergency Response Drill An emergency response drill was held on December 8 in the NRC Emergency Response Center to test internal communications procedures for NRC emergency response personnel who would respond to an incident involving an NMSS licensed operation. Emergency Preparedness Implementation Appraisals Emergency preparedness implementation appraisals were held for the Peach Bottom and Ft. Calhoun plants. Testimony before Subcomittee on Investigations and Oversight The Director of the Office of State Programs testified on December 8 before the Subcommittee on Investigations and Oversight of the House Comittee on Science and Technology on the Agreement State program in general and the Tennessee program in particular. The hearing was concerned with allegations of violations of radiation and uranium exposure standards at TNS. Inc. i 1

Copies are being provided i by PG&E to the Comissioners and the service list. l ~ Crystal River Unit No. 3 -(CR;31 .g:. Crystal River Unit 3 is presently shutdown for refueling and maintenance. i Surveillance p'rocedures (HPSI test) prior to Cycle 4 startup involved operation of primary system Make-Up pumps. At 1323 on December 3,1981, operating Make-Up pump (1B) was severely ' damaged after its suction valve in the let-down line automatically closed; automatic closure of this suction-l valve is normal for this test. The surveillance procedure is now known to be inadequate-in that the procedure did not recognize the implication of - i automatic suction valve closure during the HPSI test. Theutilityis. evaluating the surveillance test procedure to determine what was different - this time that caused the damage. Repair of the 1B Make-Up Pump is proceeding and HPSI test procedures have been modified. CR-3 is allowed to operate, however, with two out of the three Make-Up pumps operating.' Therefore, this event is not expected to prevent CR 3 startup now scheduled l for early next weitk. i j ~ TMIPO Activities { ~ s f Met-Ed has established a Technology Assessment and Advisory Group (TAAG) which will p,rovf*de independent technical assessment and advice on proposed decontamination and.defueling activities -i i at TMI-2. The TAAG will be headed by William Hamilton, formerly Director of the Bettis Laboratories, and will. utilize individuals i from government laboratories and industry with relevant expertise. j Funding support will be provided by the I)epartment of Energy. The applicant described the use of human factor engineering to the design of the CRBRP control. room. The applicant then discussed the use of a full scale control room mockup. The mockup was used to simulate operator, y responses to: CRBRP general operating instructions, emergency operating instruc-tions, CRBRP annunciator responses., and PSAR Chapter 15 events. A total of 200 walkthroughs were conducted on the control room mockup. i The applicant concluded the presentation with a comparison of the CRBRP control room design efforts to the recently issued NUREG-0700. Liquid effluents from the TMI site ' released to the Susquehanna River after processing, were made within the regulatory limits and in accordance with NRC requirements a'nd City of Lancaster, Agreement dated February 27, 1980. - ~ During the period December 4,1981, through December 10,1981, the effluents contained no detectable radioactivity at the discharge point and individual effluent sources which originated within Unit 2 contained no detectable radioactivity. l 2. Environmental Protection Acency (EPA) Environmental Data. Results from EPA monitoring of the environment around the TMI site were as follows: 3 The EPA measured KR-85 concentrations (pCi/m ) at several i environmental monitoring stations and reported the following results : l Location November 6 - November 20, 1981 3 .(pCi/m ) Gol dsboro 22 .0bservation Center 23 l Middletown 30 Yorkhaven 30 l All of the above levels of Kr-85 are considered to be background l evel s. NRC Environmental Data. The Environmental Protection Agency's counting system on which the NRC's air samples are counted is out of service for repairs. Analytical results will be reported in a future weekly status report when they become available. 4. Licensee Radioactive Material and Radwaste Shipments. On Monday. December 7,1981, three packages each containing one liter samples from the Unit 1 waste evaporator condensate storage tank were mailed to Teledyne Isotopes Westwood, New. Jersey. On Monday, December 7,1981, an EPICOR II dewatered resin liner (F-14) was shipped to U.S. Ecology, Hanford, Washington. On Tuesday, December 8,1981, an EPICOR II dewatered resin l liner (F-15) was shipped to U.S. Ecology, Hanford, Washington. On Wednesday, December 9, 1981, 20 metallic boxes of Unit 1 l LSA waste were shipped to U.S. Ecology, Hanford, Washington. On Friday, December 11, 1981, an EPICOR II dewatered resin liner (F-16) was shipped to U.S. Ecology, Hanford, Washington. 1 On Friday, December 11, 1981, two Unit 1 solidified evaporator bottoms were shipped to U.S. Ecology, Hanford, Washington. Pkjor Activities 1. Submerged Demineralizer System (SDS). Processing of batch number 12 was completed on December 9,1981. During December 9,1981, to December 11, 1981, approximately 50,000 gallons of reactor coolant bleed tank water (batch' number 13) were transferred to the SDS feed tanks in the fuel handling building. To date, approximately 290,000 gallons of reactor building sump water have-been transferred from the reactor building sump and processed. Transfers of reactor building sump water are expected to continue following the processing of batch number 13.
- 2.. EPICOR II.
Processing of SDS effluent through the EPICOR II system continued this week. Approximately 290,000 gallons of reactor building sump water have' been polished. 3 Reactor Building Entries. Raattor building entry 24 was completed on December 9,1981. Tasks were performed in preparation for the gross decontamination experiment. Specific tasks accomplished were (1) placement of splash covers over varinus equipment including the Power Operated Relief Yalve (PORV) and 'the block valve, (2) an inventory of transient combustible materials, (3) repair and repositioning of closed circuit television cameras (4) fixed point radiological surveys and air sampling, and (5) placement of thermo-luminescent dosimeters and iodine samplers. The. entry scheduled for December 11, 1981, has been cancelled. The next entry is tentatively scheduled for. December 15, 1981. 4 Emergency Drill. On December 10, 1981, the licensee conducted the annual exercise of the TMI-2 Emergency Plan by simulating a site emergency. NRC onsite staff also participated in the drill. Preliminary results from the. licensee conducted critique indicate a satisfactory drill. \\ d ENCLOSURE B. i =
l -4' I Meetings Held The NRC's Advisory Panel for the-decontamination of TMI Unit 2 met on December 10, 1981, in Lancaster. The major topic of discussion was' the f.inancial problems that are hindering the pace of the cleanup of i Unit 2. Presentations were made by GPU, NRC and EPA on the status of their activities. Comments on the progress of variout proposed solutions to the financial impasse wer,e presented by GPU, ' Edison Electric Institute i (EEI) TMI Alert and Lancaster Environmental Action Federation (LEAF). The next meeting will continue the discussion on the financial situation. Future Meetings l 1. On Friday; February 26, 1982, Lake Barrett will be speaking for the t l dinner, meeting being held by the Engineers Week Joint Planning Council l .to honor Lehigh Valley's Engineer of the Year and Young Engineer of the Year. 1 2. On Saturday, March 13, 1982, Lake Barrett will address the Society of. Manufacturing Engineers in Williamsport, PA, on the cleanup of TMI and general aspects of nuclear power. 3.' The NRC's Advisory Panel for the Decontamination of TMI Unit 2 is scheduled to meet on January 13 an'd January 28, 1982, at the Holiday Inn, 2nd and Chestnut Street, Harrisburg, PA, from 7:00 to 10:00 PM.. OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending December 11, 1981 NFS-West Valley The staff has been informed that the U. S. Court of Appeals for the Second Circuit has reversed and remanded the decision of the U. S. District Court for Western New York regarding possession and control of the West Valley site. The District Court decision, on motion by New York State ERDA, would have re-quired NFS to vacate the site to permit DOE to assme control and possession for the West Valley Demonstration Project. Licensing action taken by the l staff on September 30, 1981, would permit, but not require, the co-licensees l to transfer the site to DOE. On December 2,1981, a letter was received from NFS asking for NRC action on an application filed by NFS in October 1981. That application requested that l NRC remove NFS as a co-licensee. The application stated also that New York State ERDA would join NFS in the application. New York State ERDA has not yet joined the application. NRC has written New York State ERDA for its position concerning joining the NFS application. On December 8,1981, the U. S. Court of Appeals for the Second Circuit reversed and remanded for trial the lower court's partial samary judgment ordering NFS i to vacate the West Valley site. Thus, it would appear, NFS may remain in con-trol of the West Valley site for some still undefined period, unless they can i l come to terms with New York State. l Spent Fuel Storage NMSS, RES and NRR staff members met with Westinghouse representatives and an official from Florida Power and Light to discuss the licensing aspect of vari-ous concepts being considered for the Turkey Point nuclear power plants. Dry storage technologies using casks or dry wells in conjunction with an engineer-ed berm are being considered as well as possible construction of a new pool. No decision ha, been made on the method to be used. Present projections indi-cate that Units 1 and 2 will lose full core reserve in the existing reactor basins in late 1986 and late 1987 with a single full core reserve lost for the site by 1989. State Licenses The Industrial Section of the Material Licensing Branch recently issued li-censes to the States of New Jersey and Oklahoma (Non-Agreement States), au-thorizing temporary emergency possession of radioactive material when it has i not been adequately secured by a licensee. The states had requested this authorization so that they could take action in emergencies where radioactive material might present a public health hazard; for example, a flood or high-I way accident. The NRC staff wanted to assure that there would be no conflict between a Non-Agreement State and NRC as to how to handle an emergency. Therefore, the licenses contain a condition that the State must notify NRC immediately upon recognizing an emergency where protective action is necessary, and coordinate with NRC as to appropriate action. Items of Interest 2 Amendment Involving Medical Licensees William Walker and other senior members of the Medical and Academic Licensing Section met with representatives of the Veterans Administration on Thursday, December 3. The VA has developed a " nuclear network" that involves physician i supervision of nuclear medicine departments in remote communities via tele-l communication. The staff expressed serious concerns that this arrangement l was not described in the license applications for these hospitals and asked for further infomation in the fom of written amendment requests. The action to be taken on these amendment requests remains uncertain since the proposed arrangement does not meet NRC's definition of use "by or under the supervision of" a physician. Emergency Response Drill An emergency response drill was held on Tuesday, December 8, in the NRC Emer-i gency Response Center to test internal communications procedures for NRC emergency response personnel who would respond to an incident involving an NMSS licensed operation. Water Flow Studv i Northwestern University will be reporting the experimental results 1 or their holographic study of sprays later this month. Droplet i velocity and size distributions in sprays have been measured for tests between 1 and 3 atmospheres. Data Bank Three FLECHT SEASET tests were added to the data bank: 31615, 31701 and 31805. Also, Semiscale data in final form from Tests S-UT-6 and -7 were added and are now available to the public. ~BWR Refill /Reflood Program The 30 SSTF Data Acquisition System has been moved from Lynn, Massachusetts to San Jose, Californja and has been made operational. GE engineers are now processing 30 SSTF data in San Jose, California. 4 Preliminary results of 300 SSTF testing were presented to the ACRS on December 3,1981 in Los Alamos New Mexico. BWR FIST Facility The vessel design is complete and the thermal stress analysis conducted by INEL has uncovered no major design problems. GE has proposed l that INEL also analyze pipe loads on the vessel since INEL staff already have a computer program set up to analyze the vessel stresses. A GE design review of the vessel was attended by INEL staff and results provided to NRC. GE is now ready to place the vessel order. GE staff met with INEL staff to discuss data reduction and analysis l techniques used at INEL. We are considering reducing FIST data at INEL using existing INEL systems. INEL and GE staff are also finalizing which BWR TRAC scaling calculations are to be perfonned l by INEL. During Phase II negotiations, all ATWS tests were moved to Phase I in order to obtain early ATWS test data. This has resulted in a disconnect in facility capability versus test requirements. NRC ~ staff desire core void fraction measurements for these tests and plan 'to provide instrumentation developed at INEL to make these measurements. GE has recommended, however, that.this instrumentation not be included in Phase !. The FIST design uses an improved, but untried method, of providing power to the bundle. The proposed instrumentation must penetrate the electrically hot channel wall. Thus l GE has recommended not using this instrumentation until the new system design has been proven and experience with the system obtained. An option now being considered is to nove half of the ATWS tests back to i l Phase II. Thus we would obtain some early ATWS data during Phase'!, but not obtain all the desired data until Phase II. ' LOFT. i Work is continuing on preparations for installing the F1 pressurized center fuel assembly. There was some difficulty in mmoval of the i downcomer instrument assembly, but the problems have been corrected. The instrument assembly which has been in. place throughout LOFT testing I took longer to extract than was anticipated. The plant is expected l to be ready for startup and physics tests by mid-January. Test schedule reprograming has now targeted the L9-3 Loss-of-Feedwater 4 ATWS for late March and the L6-6 Boron Dilution from cold shutdown for early April. The test schedule and program objectives and plans will be discussed with INEL/ DOE in late December. Now that FY 1982 j funding has been received, the program plans can be finalized. 1 Semiscale System preparation for the NRR requested test UT-B is in progress (test of the Westinghouse level device followed by a pump restart transient). primary loop leakage tests have been satisfactory 1 and instrument ranging and testing are completed. Themal insulation replacement is in progress and pretest loop preparation is underway. B&W has all of the information they need to study use of Semiscale i to test their level instrumentation. They are considering possible test configurations and connection points. planning is continuing for a steam generator secondary side upset j test series to be conducted in the spring (1982) to provide data j on overcooling and overpressurization issues. Special short-tem tests are being evaluated to provide pressurizer and candy cane (B&W) type data needed for calculation of overcooling transients. 2 Fuel Behavior Branch 1 l The ORNL clad ballooning separate effects test, MRBT-6, was successfully perfomed December 3. The MRBT-6' assembly consists of an 8 x 8 array of i internal-electrically-heated "15 x 15" type Zircaloy cladding tubes. The 1 64 simulated fuel rods were ruptured at an average burst temperature of j 1205K (932"C or 1710"F) in the et + 5 phase, reduced clad-strain " valley." 1 The average burst pressure was 2.85 MPa (400psig). The cladding temperature i ~ ramp rate at the time of rupture was about 3.5K/second. The initial visual examination of two faces of the test bundle showed burst strains of perhaps 40%. Most of the cladding diametral increase occurred in a 6" to 8" rod l 1ength downstream from the midpoint between adjacent grids. The strain loca- ] tions were similar to those seen in the three NRU in-reactor clad ballooning tests. i ENCLOSURE E I

'Ihis clarification was made necessary l by the Department of Transportation's deletion from its regulations of l a section which has been referenced in 540.13(c)(6). The anendnent replaces tne reference to the set of shipping container requirements that were contained in 8178.250 of DOT regulations by a single requimment which has been selected fma that set. This single requirement for minimum cladding i of the uranium shielding constitutes no substantive change from the expected cladding when the entire 3178.250 was incorporated by reference. The issue was raised by an intervenor who sought to i have the application dismissed with prejudice. j San Onofre Generatino Station, Units 1 and 2 On December 8,1981, the Commission issued a Memorandum and Order (CLI-81-33) in which it decided that current NRC regulations do not require consideration of the impacts on emergency planning of earthquakes which cause or occur during an accidental radiological release. Whether or not emergency planning require-ments should be amended to include these considerations is a question to be addressed on a generic basis as opposed to case-by-case basis. l ITEMS OF INTEREST OFFICE OF INTERNATIONAL PROGRAMS WEEK ENDING DECEMBER 11, 1981 IAEA PWR Seminar in Vienna, Austria A special IAEA Seminar on the Safety of Two-Loop Pressurized Water Reactors was held in Vienna, Austria from November 16-20. The meeting was attended by 52 participants from 11 countries, including the developing countries which have imported Westinghouse 2-loop PWRs (Brazil S. Korea, the Philippines and Yugoslavia). S. Varga, NRR, led the U.S. delegation. Westinghouse, INPO, Wisconsin Electric Power Corporation and United Engineers and Contractors Inc., were also represented. Foreign Visits to NRC On Monday Mr. Per Unckel, a Member of the Swedish Parliament, met with Chairman Palladino and Joseph D. Lafleur, IP Deputy Director, to discuss changes in NRC's licensing process. On Monday Mr. Takeshi Ikushima of the Japan Atomic Energy Research Institute (JAERI) met with R. Odegaarden of the NPSS Transportation Certification Branch to discuss computer codes for the safety assessment of transportation casks. Mr. Hiroshi Mizuta of the Japan Institute for Nuclear Safety (JINS) met with S. Bernstein and T. Eng of the RES Transportation and Materials Risk Branch and with A. T. Clark of the NFSS Advanced Fuel and Spent Fuel Licensing Branch to discuss computer codes for the safety assessment of fuel cycle facilities. Mr. Fujish'ge Yamauchi of JINS met with T. Wolf of AE0D to discuss computer code data base management. The JINS was organized in August 1980 as an institute of the Nuclear Power Engineering Test Center, under the sponsorship of the Japanese Federal Ministry of International Trade and Industry (MITI) and the Science and Technology Agency (STA), for the purpose of developing safety l analysis codes to be used in reactor and fuel cycle facility licensing and seismic studies. l Meeting with Combustion Engineering on Exports to Mexico On December 10, IP staff met with representatives from Combustion Engineering, Inc., at their request, to discuss U.S. Government export control requirements under the Nuclear Non-Proliferation Act of 1978 for exports of nuclear equipment, fuel and technology to Mexico. The hearing was concerned with allegations of violations of radiation and uranium exposure standards at TNS Inc., a Tennessee licensee i that manufactures depleted uranium projectiles for the Department of Defense, j On May 1, 1981 workers represented by the Oil, Chemical and Atomic Workers Union at the plant struck due to alleged violations of health guidelines for exposure to uranium. The company has since hired non-union labor and the plant has remained partially functional. The purpose of the hearing was to l examine the circumstances of the dispute and how well the State and Federal agencies are carrying out their oversight responsibilities. Congressman Gore suggested that TNS acknowledge the union safety concerns, improve working conditions at the plant and hire back the striking workers. He suggested that i all occupational safety be regulated at the Federal level. The union supported l the suggestion. There was a strong recommendation for the State to adopt civil i i penalty legislation. The State plans to seek legislation for such authority. i Colorado's Governor Lam has included the creation of a low-level radioactive waste site in Colorado among the tentative issues that he wants the General l Assembly to consider in 1982. These issues are being circulated among legis-lative leaders for comments. Colorado has spearheaded the creation of the { Rocky Mountain Low-Level Radioactive Waste Ccmpact Group. \\ \\ Steve Salomon attended a symposium organized by NRC and Oak Ridge National i Laboratory on December 8 and 9, 1981, on the site suitability requirements of l low-level waste disposal as defined in 10 CFR Part 61. Of much concern to the l generators of low-level waste is the Congressional deadline of January 1, 1986. i According to the 1980 Low-Level Radioactive Waste Policy Act (P.L. 96-573), after that date any interstate compact on low-level wastes may restrict the-l, use of.the regional. disposal facilities under the compact to the disposal of l low-level radioactive waste generated within the region. Because of the time necessary to form a compact, site a disposal facility, license and construct it, some participants at the symposium are concerned that no region that does not already have a site will be able to meet the deadline. Therefore,.some States risk the possibility of being excluded by compacts that already have an operating disposal facility. 1 Robert Trojanowski, Regional State Liaison Officer, Region II, made a presentation J on December 10, 1981 on emergency planning and participated as a member of an Emergency Planning and Response Panel at the Georgia Institute of Technology. Menczer, Regional State Liaison Officer, Region III, was in Kansas City, M0 on December 11, 1981, meeting with the Regional Assistance Comittee, Region 7 to discuss and consolidate comments on the Calloway County Emergency Plan. This evaluation will be utilized by the Federal Emergency Management Agency in its preparation of an interim finding on Calloway which is due January 1, 1982. Following the meeting, Mr. Menczer met with FEMA Region 7 staff to discuss new developments in NRC's Regulation of Emergency Preparedness. On December 8,1981, Dean Kunihiro, Regional State Liaison Officer, Region V, met with the Santa Clara County Mass Casualty Comittee to provide information and assistance in planning for their annual medical exercise which is scheduled for March 9,.1982. The scenario calls for a simulated transportation accident of radioactive materials with forty casualties, some of whom would be radio-actively contaminated. On December 9, 1981, Dean Kunihiro met with the tri-county emergency planning committee to discuss offsite emergency preparedness plans for the Rancho Seco' nuclear power station. The discussion included the history i of the LER rulemaking, the specific criteria contained in the draft proposed l rule, the Canmittee to Review Generic Requirenents (CRGR) comments on the i draft proposed rule, and anticipated schedule for preparation of the final rule. As part of the staff's continuing effort to work closely with INP0 concerning the development of this proposed rule, representatives from INPO attended the subcomnittee meeting and actively participatedin the discussion. I Subsequently, on December 11, 1981, the staff met with the full ACRS to discuss the same matter. The subconnittee and the full committee were both very supportive of the staff's efforts to improve the LER reporting i requirements. During the initial i heat-up on December 11 a leak from the primary coolant system of 32 gpm at 2000 psi and 480* occurred. i Oyster Creek On December 9, while at full power, a steam leak from an Isolation Condenser isolation valve was detected. An inspection showed thread damage on the drive mechanism to the valve stem. The plant vas taken off line for an estimated 2-week inspection / repair outage. TMI-1 The special hearing into operator cheating and related matters was completed on December 10, 1981. The proposed findings are scheduled for January and the Special Master's report is expected in early March 1982. l On December 14, the presiding ASLB issued a Partial Initial Decision on plant I design and procedures, separation and emergency planning issues. It found that TMI-1 could be operated provided the licensee satisfies a number of specified requirements. It also found that the pendency of the reopened issue of cheating should not preclude short-tenn operation of TMI-1 at power levels not to exceed. five percent of design power. TMI-2 Reactor Building Entries ( During reactor building entries this week, an extensive characterization of I surface contamination was completed in preparation for the decontamination experiment. Surface contamination samples were taken from 48 locations in the reactor building. Decontamination experiment water washes inside the reactor building are scheduled to conrnence in mid-January 1982. Spurious Siren Activation in the Vicinity of TMI On December 17, several early warning sirens in the vicinity of TMI sounded inadvertently. The licensee reported that the siren activation was caused by a decoding device malfunction. -4

During the outage, in addition to the normal refueling activities, maintenance, and surveillance testing, the following significant modifications were perfomed: Auxiliary Feedwater System Modifications to" prevent water hammer and to increase reliability; Reactor Pressure Vessel. Vent; Wide range steam generator level 3 indication; Long Tem cooling-boron precipitation prevention modification;- 1 Fire protection modifications; Electrical equipment qualification testing; j Radwaste Modifications; New station batteries to double capacjty; Automate ~ j auxiliary feedwater start; Water level indication for condensate storage tank; j r Seismic anchorages for piping and electrical equipment; NUREG-0737 post i accident shielding. j During the initial heat-up on December 11,'1981 a leak from the primary coolant. 5 system of 32 gpn at 2000 psi and 4800F occurred. The charging pumps (2 contisuous,1 intemittent) were able to keep up with the leak. The reactor j was still in the pre-critical stage of start-up and had not operated since l August. Therefore, there was no significant release of radioactivity from the reactor coolant system. An " unusual event" was declared in accordance with the Emergency Plan and all notifications,were made. After the plant was. depressurized and cooled down, it was detemined that in replacing a control ] rod drive seal package, alorkman.had failed "to insta-1 a'flexital-Jic' gasket.. This was rectified and the plant heat-up was restarted on December 14, 1981. 4 Criticality is now scheduled for December 16, 1981. Oyster Creek 1 On December 9,1981, while at full power, a steam leak from an Isolation Condenser isolation valve was detected. The noma 11y open valve was manually closed. Inspection of the Limitorque operato'r showed thread damage on-the 4 1 drive mechanism to the valve stem. Review of the problem indicated that limit } bypassing to backseat the valve is the probable cause. Six other motor operated-Isolation Condenser valves, and 2 cleanup system valves may have similar damage. The following activities were completed during the outa.ge: a successful' - containment integrated leak rate test, remaining 120 month.in-service ~ inspection items; test sleeving of 12. tubes in the-B 'stsam generator. 4 x of which had i been previously. plugged due to the presence of defects and replacement of bothi ~ low pressure turbine rotors. Summary of CRBR Review Meetinos A meeting was held on December 10,1981 in Bethesda, Maryland'to discuss emergency preparedness for the Clinch River Breeder Reactor Project. The applicant stated that planning for CRBRP emergency preparedness is based on: 10 CFR, Appendix E; NUREG-0696; NUREG-0654; NUREG-0718; NUREG-0814; and Regulatory Guide 1.101. The applicant discussed the technical support center, the operational suppert center, the emergency operations facility, and the use of meterological data. The applicant stated that evacuation time estimates have been completed and that the probabilistic risk assessment had been issued in 1977 and will soon be updated to include more recent reliability data. The applicant stated that section 13.3 of the PSAR " Emergency Planning" is being ugdated and this revision will be sub-mitted in February.1982. A meeting was held on December 14, 1981 in Bethesda, Maryland to discuss CRBRP safety related instrumentation and control systems. The applicant described the reactor and plant systems as documented in Chapter 7 of the PSAR. The I presentation included: the ~ design concepts and requirements; the design criteria and their implementation; a discussion of the shutdown heat removal systems; and, a discussion of the reactor plant auxiliary systems. The applicant also discussed supporting plant control systems as well as instrumentation which is unique to LMFBR's. the SDS. To date. 290.000 gallons of reactor building sump water have been processed. SDS perfomance parameters for batch number 13 l are attached. 2. EPICOR II. EPICOR II processing of SDS effluent continued this l week. The latest performance parameters for EPICOR II are attached. I 3. Reactor __ Bui_1 ding Entries. Reactor building entries 25 and 26 were completee on December 15 and 17.1981. During these entries an i extensive characterization of surface contamination was completed in preparation for the decontamination experiment. Surface con-tamination samples were taken from 48 locations in the reactor buil ding. The sampling technique included a vacumning device for l j collecting loose surface contamination and a boring machine in conjunction with the vacuum for collecting fixed contamination". i i Reactor building air samples were taken; a survey was made of certain areas where there was evidence of,high temperatures during i l the accident. Reactor building entries will continue after the holidays. Decon-l tamination experiment water washes inside the reactor building are scheduled to commence in utd January 1982. 4 4 Spurious siren Activation. On December 17, 1981, between 5:00 and j 6:00 PM several early warning sirens in the vicinity of TMI sounded inadvertently. The licensee reported that the siren activation was } caused by a decoding device malfunction. .~ _ l 1 1 i 3 0FFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS 1 Items of Interest Week Ending December 18, 1981 1 i ~ Stepan Chemical Company On December 11 a meeting was held with representatives of Stepan Chemical Company to discuss what additional information was needed for our review of the amendment application to move the thorium contaminated soil from the Ballod Associates property in Maywood, New Jersey, for storage under the Stepan Chemics 1 Company's license. The Stepan representatives were infomed that a description was needed of their procedures for removing and transporting the residues to the Stepan property as well as the actions they would take to protect the workers and the eavironment at both the Ballod j site and the Stepan storage site. Also needed was the concentrations of a the various radionuclides in the material that is being moved and the leach-ability of these materials with solutions having a pH of between 5 and 8. The Stepan representatives stated that Ballod Associates desired to move the i material some time between now and the first of the year. The staff in-1 fomed them that with the holiday season coming up, it is doubtful that our contractor (ORAU) who will perform a radiological survey to verify that the l site meets release criteria could be scheduled on such short notice; how-ever, if Stepan would get the requested information to us, the staff would do everything possible to expedite the review of the application. I W. R. Grace i On December 14 staff members of NMSS ELD, IE Headquarters and Region I met with representatives of the Department of Energy to discuss which group l would be responsible for the survey of the W. R. Grace property in Pompton Plains, New Jersey. Rare Earths, Inc. of Pompton Plains, New Jersey (now owned by W. R. Grace), processed monozite ores to extract thorium for the Atomic Energy Comission and rare earths for public sale. The processing for the Atomic Energy Commission was initiated in 1948 and continued until 1954 when an AEC license was issued for this processing. The license was temirtt::: 16 Unuary 1975. It is on a list of approximately 40 sites formerly licensed which 6re being evaluated to detemine if a rom-prehensive radiological survey is necessary. A recent survey by Region I indicates that there are areas onsite as well as along a creek adjacent to the property that have concentrat(ons of thorium that exceed our current guidelines for unrestricted use. Accordingly, a comprehensive radiological i survey will be perfomed to detemine what, if any, remedial actions need to be taken. I Westlake Landfill As has been reported previously, residues from the former Cotter Corporation operation at Latty Avenue in Hazelwood, Missouri, were transferred to the Westlake Landfill at Bridgeton, Missouri. NRC has recently received a draft of a radiological survey performed by our contractor, Radiation Management ENCLOSURE C
Items of Interest Corporation. Some of the conclusions of this report were the subject of recent newspaper articles in the St. Louis Post Dispatch and St. Louis Globe Democrat. There has been considerable public interest in what actions the NRC may require at this site. T.he draft report will be reviewed with the State EPA, and the property owners prior to NRC deciding what, if any, remedial action is necessary. Meeting with Gesellschaft fuer Nuklear Service A meeting with NMSS staff on December 15, 1981, Gesellschaft fuer Nuklear Service, mbH (GNS) staff confirmed that the GNS dry spent fuel storage cask standard design topical report will be submitted in January 1982. Licensing Workshop On December 14, 1981, the Medical and Academic Section of the Material Licen-sing Branch held a workshop at Region I Headquarters on the issuance of broad medical licenses. Senior reviewers of the Medical and Academic Section. Material Licensing Branch, members of the Region I Licensing Staff and the Section Leader from Region III were in attendance. Purpose of the workshop was to discuss methods and policies for the issuance of broad medical li-l censes. Visit to Babcock and Wilcox Members of the Division of Safeguards are visiting Babcock and Wilcox in Lynchburg, Virginia. The purpose of the visit is to assess the feasibility and impact of implementing the provisions of the advance notice of proposed rulemaking for the MCLA Reform Amendments. Similar visits to all affected licensees are planned. Review of Program of Technical Assistance to the IAEA Representatives of NMSS, ACDA, State and DOE, members of the Technical Sup-port Coordinating Committee (TSCC), met with the IAEA during the week of December 6,1981, to review the the IAEA's preliminary requests for assistance in FY82. The results of the meeting will determine whether the presently planned instrumentation will meet the requirements and what measurement range the instruments should be designed for. The contract for the UPTF construction has finally been signed ~ between GRS and KWU. 6. LOFT The work schedule for refueling has been reworked to allow'for additional inspection of the downcomer instrument stalk area. Problems with removal of the original instrument stalk has made it necessary to perform additional checks for protrusions or reasons for the stalk to hangup during removal. Work originally planned for the post-refueling period is now being performed prior to F1 bundle insertion. The change should keep the L9-3 ATWS test on schedule for the end of March. The test date critical path' is receipt and installation of new code safety valves and PORY actuator hardware. Marviken Aerosol Transport Test Proposal Review i In September 19B1. EPRI, Ontario Hydro, KEMA (The Netherlands), and Studsvik Energiteknik developed a preliminary proposal for conducting reactor primary system, volatile fission product, and aerosol transport behavior tests at the Marviken facility in Sweden. RES/FBB is currently reviewing this proposal for possible NRC participation in the project. Steps in the review process undertaken to date include: a) A preliminary review of the draft proposal, by BCL, ORNL,' and RES/FBB. b) A review meeting at ORNL, October ?> and 16, involving BCL, ORNL, and RES/FBB. c) Discussions with EPRI (Wall and Vogel) on November 20. d) A 2-day project review meeting at ORNL (December B and 9) with EPRI. LMFBR/CRBR Irradiated-Fuel-Disruption Experiments The first two irradiated-fuel experiments in the joint NRC-KfK (German) program on fuel-failure and fuel-disruption (FD) under Loss-of-Flow (LOF) accident conditions in a LMTBR were successfully performed in the ACRR test reactor at Sandia National Laboratory. Xn these experiments, sec-tions of clad fuel pins in argon gas (simulated sodium vapor) are exposed to calculated LOF power histories in the ACRR experiment cavity, and tha resulting cladding and fuel failure, swelling, and disruption, and the surface temperatures are measured by high-speed cinematography. The results are directly applicable to the CRBR review, and they form a basis for development and verification of fuel behavior models in LMFBR accident Fuel and clad disruption and displacement are important analysis codes. both for the initiation-phase accident sequences and for the transition-The NRC-supported FD-2 experiments are for LOF phase initial conditions. conditions. in the new CRBR heterogeneous core, and the KfK supported FD-4 experiments are for the LOF conditions in the German SNR-300 hemogeneous core, which is similar to the old CRBR core which NRR previously reviewed. The complementary FD-2 and FD-4 results add to technical understanding of the phenomena. Following the initial " peel off'" of the molten clad in both these experi-fuel j ments, the hypothesized strongly dispersive fission-gas-driven i disruption did not occur, even though the energy deposition in both cases j was about 2500 J/gm. With the 90% areal melting in the CRER low-power transient from steady-power conditions, the fuel remained intact, but had very-large (about 40% areal) swelling. 'In the higher power SNR-300 transient to the same energy deposition, week, viscous disruption occurred upon surface melting, with subsequent slumping of the liquid fuel. Detailed analysis of the results is underway, and the next experiments are scheduled for January. 4. Three Mile Island Technical Evaluation Group R. Van Houten, Fuel Behavior Branch, RES, represented the USNRC at the December 9 meeting of the Three Mile Island Technical Evaluttion Group. Restart On December 14, 1981, the presiding ASLB issued a Partial Initial Decision on plant design and procedures, separation and emergency planning issues. In this 800 plus page decision the ASLB found that TMI-1 could be operated provided the licensee satisfies a number of specified requirements. The ASLB also found that the pendency of the reopened issue of cheating should not preclude short-term operation of TMI-1 at power levels not to exceed five percent of design power. Three Mile Island, Unit 1, Restart On December 15, 1981, the presiding ASLB issued a Memorandum and Order in which it concluded that there was no need for further evidentiary hearings on any of the contentions relating to the adequacy of the Environmental Impact Assessment or the need for an Environmental Impact Statement. OFFICE FOR ANALYSIS AND EVALUATION OF OPERATIONAL DATA ITEM OF INTEREST WEEK ENDING DECEMBER 18, 1981 Case Study on Loss of Service Water at Calvert Cliffs Following completion of peer review, the AEOD case study on the Calvert Cliffs May 20, 1980 loss of service water was issued in final on December 17, 1931. This event involved the loss of both redundant trains of the safety-related service water system when the system became air bound as a result of the failure of a nonsafety-related instrument air compressor aftercooler. The consequences of this event were minor. However, this event, involving the failure of a single nonsafety-related component causing the disable-ment of both redundant trains of the safety-related service water system, is nonetheless significant since it involved two fundamental aspects considered in the design of safety-related systems: 1. Interaction between safety and nonsafety-related systems and components; and 2. Cormon caused failure of redundant safety systems. The review of this event revealed no immediate safety concerns; however, AEOD believes there is a need to reevaluate the following: the assumptions used in analysis of the steam generator tube rupture for Calvert Cliffs and possibly other PWRs licensed prior to the " Standard Review Plan"; the assumption of atmospheric dump valve operability on two-loop PWRs having one atmospheric dump valve per steam. generator.following a stesn generator tube rupture; and for the isolation provisions at the interface between the safety and nonsafety-related portions of the service water system at Calvert Cliffs. Ginna An ongoing ECCS reanalysis by Rochester Gas and Electric Corporation (RG&E) has revealed a non-conservative error in assumptions regarding containment heat removal and its effects on LOCA blowdown and peak clad temperature (PCT). Although four containment fan coolers are available, only one was assumed in the original analysis. If all four are assumed to operate, the containment back-pressure is less, blowdown occurs quicker, and the core is uncovered a longer period of time. Initial estimate is that, although PCT will be higher than 0 1922 F as originally calculated, it should not exceed 2200 F as allowed by 0 10 CFR 50.46. While the RG&E calculations are being completed, the NRR' staff and the Resident Inspector are monitoring'the situation closely. Coerator Licensino i During the month of November,1981 the Operator Licensing Branch issued 69 new Reactor Operator Licenses and 46 new Senior Reactor Operator Licenses. These licenses were issued to personnel at 7 facilities. In addition, the OLB issued 15 renewal Reactor Operator Licenses and 34 renewal Senior Reactor Operator Licenses. These licenses were issued to personnel at 13 facilities. inree vile Island Unit 2 Metropolitan Edison Company suomitted a proposal to perform a decon-tamination experiment of the Unit-2 reactor building. The primary purpose of the experiment is aimed at reducing radiation levels attributed to surface contamination. A water spray or flush, using sater pressures ranging f rom 60 psig to below 10,000 psig and temperatures from ambient to 140' F will be tested. l Based on a review of the proposed experiment, the staf f finds the l licensee's proposal acceptable subject to the staff's approval of the licensee's procedures to implement the experiment. This was the result of manipulating main salt water circulation controls and a sub-l sequent condensate pump trip - because of low condenser (condensate) water level. A total loss of feedwater followed. The main steam iso-I l lation valves were closed manually until feedwater flow was re-established. ECCS or emergency isolation condenser was not activated. Humboldt Bay Power Plant Unit No. 3 On December 28, 1981 an exemption was granted to Pacific Gas and Electric Comoany from the " calendar quarter" definition provided in 10 CFR 20.3(a)(4) as it applies to the Humboldt Bay radiation exposure assessment program. The exemption permitted the licensee to shorten the final 1981 calendar cuarter to less than 12 weeks so that the first quarter of 1982 begins January 1, 1982 rather than January 15, 1982 as would othemise be the case. Previously, a Notice of Violation and Proposed Imposition of Civil Penalties in the amount of $1,000 was issued based on alleged violations involving failure to perform adequate surveys that resulted in an uptake by an employee into the body of iodine-125 in excess of NRC limits. After consideration of the licensee's response, the IE staff concluded that, with the exception of or,e Severity Level VI violation for which no civil penalty has been proposed, the violations did occur and no adequate reasons were given by the licensee for miti-gation or remission of the civil penalties. b. OFFICE OF THE EXECUTIVE LEGAL DIRECTOR ITEMS OF INTEREST Weeks Ending December 25, 1981 and January 1, 1982 Midland Plant, Units 1 and 2 On December 22, 1981, the presiding ASLB issued a Partial Initial Decision in this construction pemit remand proceeding involving the issues of radon releases and charges relating to conduct of parties. The PID concluded that the radon effects from uranium fuel supply would be negligibly small compared to the effects of natural radon emissions, and are therefore not significant. It also concluded that the questions raised as to the conduct of parties and counsel in the original suspension proceeding have now been fully aired and resolved in compliance with the Appeal Board mandate and no sanctions are necessary or appropriate. Three Mile Island Nuclear Station, Unit No.1 On December 23, 1981, the Commission issued an Order (CLI-81-34) in which it extended to January 13, 1982 the date on which comments from parties are due on the December 14, 1981 Fortial Initial Decisions of the presiding ASLB regarding hardware design issues, emergency planning and the separation of Units 1 and 2. In addition the Commission determined that the Appeal Board would not have authority in this proceeding to act upon requests for a stay of the ASLB decision. Clinch River Breeder Reactor Plant f On December 2t,1981, the Commission issued a Memorandum and Order (CLI-81-35) in wnich it established procedures for considering the merits of the Department or Energy's request for an exemption from 10 CFR 50.10 in order to begin site preparations. The Commission determined that the request would be considered in an informal proceeding involving written comments and oral preparations to the Comission itself. The participants to this proceeding will be the applicants, NROC, Sierra Club and any other interested person. The NRC staff will not be a ,;orty to this informal proceeding involving the exemption request. Comanche Peak Steam Electric Station, Units 1 and 2 On December 29, 1981, the Commission issued an Order (CLI-81-36) with respect to the presiding ASLBs retention of a dismissed intervenor's contentions pursuant to its sua sconte authority under 10 CFR 6 2.760a. ITEMS OF INTEREST OFFICE OF INTERNATIONAL PROGRAMS i Weeks Ending December 25, 1981 and January 1, 1982 i U.K. Geological Research on High-level Wastes { After reviewing the British research program on long-term options for disposing of high-level radioactive waste, the Radioactive Waste Management Advisory Committee of the U.K. has recommended that serious consideration should be given to the possibility of storing high-level _ waste on the surface in solid form for a period of 50 years and maybe longer. At the end of that period, a decision would be made on whether to continue to store it or to bury it under-l ground. Regarding alternative storage methods for high-level waste, the U.K. l 1s following closely the studies being done in other countries. l Reouest by Switzerland for Use of Separated U.S. Plutonium in EURATOM l IP has received a subsequent arrangement request from DOE involving proposed U.S. approval of the use of plutonium separated in France from U.S. origin spent fuel. Under the proposed arrangement, the approximate 58 kilograms of i plutonium would be used either in the Super Phenix Reactor in France or in the SNR 300 breeder reactor in West Germany. NUREG-0030. The new book has been greatly sim-plified, eliminating the extensive PERT networks in favor of 13 critical milestone dates on critical bulk commodities that lead to fuel loading. The new Yellow Book has been automated by Bill Lovelace and Janie VanEpp. This and future editions will be produced by computer. r OFFICE FOR ANALYSIS AND EVALUATION OF OPERATIONAL DATA ITEM 50F INTEREST WEEKS ENDING DECEMBER 25, 1981 and JANUARY 1,1982 Commission Briefino on NRC's Operational Experience Analysis Program AEOD together with IE, NRR, RES, and NMSS briefed the Canmission on December 21, 1981 on the overall NRC program for the collection, assessment and feed back of operational experience. Each office provided a summary of its responsibilities and review products involved with the analysis of reactor events and operating experience. It was estimated that the staff is currently spending about 70 professional staffyears and between two and three million dollars on this activity. AEDD presented a brief description of its analysis program, including the use of the Sequence Coding anc Search System (SCSS) as an aid in the identification of trends and patterns in operational experience. Past and current trend studies have been severely hampered by the inability to retrieve data from current LER data systems with precision, completeness, or efficiency. The SCSS will overcome these deficiencies and will facilitate future trends and patterns studies. Computerized Storace of Nenreactor Operational Experience Data Testing of an ORNL nonreactor operational experience data base was started on Monday, December 14, 1981. ORNL has 143 reports entered, and 32 additional reports have been forwarded to them. ORNL is working on an output format based upon an AEOD sample. Af ter the data base has been satisf actorily tested, event descriptions for nonreactor events that have occurred in prior years will be added. Additionally, the possibility of transforming an Argonne nonreactor data case to the ORNL data base will be explored, thus linking data from two canplementary data bases covering different time periods. If the data bases can be linked,all available data on nonreactor events will be accessible from a single, on-line system. AEOD met with representatives of NMSS and IE to discuss the AEOD data base of misadministration reports. Efforts are underway to work with the other offices so that their needs regarding medical mis-administration reports can be met with the AE00 data file. l l l l l ENCLOSURE K l i
