Section 4 of 01-01, Operating Principles and Philosophies, Revision 32, which states the operating philosophy regarding the use of procedures, requires that all plant evolutions, except simple evolutions, be conducted in accordance with approved plant procedures.
If a procedure does not exist, the plant is to be placed in a safe condition and the appropriate procedures developed and approved.
Contrary to the above, written procedures for local operation of. a reactor feed pump were not developed and implemented. The failure to have such a procedure for the removal from service of the 2A reactor feed pump on April 16,1990, resulted in a reactor vessel level perturbation, recir-culation pump runback and entry into instability region C.
This is a Severity Level IV violation (Supplement 1).
Pursuant to the provisions of 10 CFR 2.201, Carolina Power and Light Company is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN:
Document-Control Desk, Washington, DC
- 20555, with a copy to the Regional Administrator, Region 11, and a copy to the NRC Resident inspector, within 30 days of the date of the letter transmitting this Notice.
This reply should be clearly marked as a " Reply to a Notice of Violation" and should include:
(1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.
