NOTICE OF VIOLATION Rochester Gas and Electric Docket / License No. 50-244/DPR-18 Corporation During inspection on May 1-31, 1982 and in accordance with NRC Enforcement Policy, 10 CFR 2. Appendix C (47FR9987), March 9,1982), the following violation was identified.
10 CFR 50, Appendix B, Criterion X.
Inspection, states in part, "A program for inspection of activities affecting quality shall be established and executed... to verify confor-mance with the documented instructions, procedures and drawings for accomplishing the activity."
Administrative Procedure (A)-1001, Inspection and Surveillance Activities Revision 9.
17, 1981, paragraph 3.5, states that Quality Control (QC) inspectors perform-September The ing surveillance shall document negative findings on a QC Surveillance Report.
inspector is then to indicate whether corrective action is required, and submit the report to QC supervision for review and approval as documented by signature on the report. QC supervision then tracks reports requiring follow-up action through close out.
11,1982, QC Surveillance Report 82-310, documenting Contrary to the above, on April that a weld rod had been dropped inside the 'B' Steam Generator during modifications, was improperly filled out, in that no corrective actions were indicated as required.
Further, a QC supervision review closed out the Report four days prior to the removal of the weld rod.
This is a Severity Level V Violation (Supplement 1).
