APPENDIX A NOTICE OF VIOLATION Rochester Gas and Electric Corporation R.
E. Ginna Nuclear Power Plant Docket No. 50-244 License No.
DRP-18 During the inspection conducted on March 7
April 5,
- 1988, and in accordance with the General Statement of Policy and Procedure for NRC Enforcement Actions 10 CFR Part 2, Appendix C (Enforcement Policy 1986),
the following violation was identi-fied:
Technical Specification 6.8. 1 requires written procedures shall be established, implemented, and maintained covering activities referenced in Appendix A of Regulatory Guide 1.33, November 1972.
Appendix A, paragraph I, addresses control of maintenance.
The licensee's Maintenance Procedure M-43. 15.2, "Installation of B Steam Generator Hot/Cold Leg Prim.-ry Manway Cover Utilizing Hydraulic Stud Tensioner"
- requires, in part, all studs be verified to have exhibited an elongation of 0.0075+
.001 inch (step 5.21.23).
Otherwise, the manway cover tensioning pro-cess (steps 5.21 through 5.21.22) must be repeated.
Contrary to the above, on March 20,
- 1988, the criterion for manway stud elongation was not met for all studs and although step 5.21.23 was not com-pleted for both covers; remaining steps of the procedures were completed and
- signed, including:
5.29; guality Control verify that manway installation is complete.
5.30, Notify Control Room that B steam generator hot/cold leg manway installation is complete.
This improper action resulted in the plant being required to cool down and retension the bolts on manway covers.
C This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Rochester Gas and Electric is hereby required to submit to this office within 30 days of the date of the letter trans-mitting this Notice, a written statement or explanation in reply, including:
(1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results
- achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
Mhere,good cause is shown, consideration will be given to extending the response time.
