Report Date: February 26. 1982 Occurrence Date: January 27, 1982

. Identification of Occurrence:

A leak was discovered in letdown cooler "A", creating 1 an event reportable under Technical Specification 6.9.1.9.d.

Conditions Prior to Occurrence:

l Mode 1 power operation (100%).

Description of Occurrence:

At 0740, during normal operation, an increasing activity level was found in the service water system at RML-3. After i various component isolation, a leak was discovered in let-down cooler "A". Letdown cooler "A" was isolated. Redun-1 dancy is provided by letdown cooler "B". ,

i Designation of Apparent Cause:

The cause of this event is attributed to a tube-to-shell leak.

Analysis of Occurrence:  !

, There was no effect upon the health or safety of the general

, public and the letdown cooler is still operable and shall be i used only under unusual conditions. There was no unplanned release.

Corrective Action:

, Letdown cooler "A" was isolated and will be replaced with a spare cooler.

. Failure Data:

This is the first occurrence concerning the letdown coolers

.and this is the eleventh report under this Specification.
