Reactor Protection System Instrumentation - Inoperable.

This report was initiated by Incident Report 82-024.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 96% - Unit Load 1080 MWe DESCRIPTION OF OCCURRENCE:

On January 28, 1982, the operator noticed that the Overpower Delta T indication for Channel 21 had failed high. At 0810 hours0.00938 days <br />0.225 hours <br />0.00134 weeks <br />3.08205e-4 months <br /> the channel was declared inoperable and Action Statement 3.3.1 Action 6 was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.3.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The Signal Summator 2TM412E had no output due to failed capacitors.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.1 Action 6 requires:

With the number of operable channels one less than the total number of channels, startup and/or power operation may proceed provided the following conditions are satisfied:

a. The inoperable channel is placed in the tripped condition within one hour,
b. The minimum channels operable requirement is met; however, one additional channel may be bypassed for up to 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> for surveillance testing per Specification 4.3.1.1.

CORRECTIVE ACTION:

The channel was placed in the tripped condition within one hour.

All technical specification requirements were met. Five capacitors were replaced in the unit, and the unit was bench tested satisfac-torily. Functional test 2PD2.6.001 was satisfactorily performed.

At 1345 hours0.0156 days <br />0.374 hours <br />0.00222 weeks <br />5.117725e-4 months <br /> Channel 21 was declared operable, and Action Statement 3.3.1 Action 6 was terminated.
