o SUPPLEMENTARY'INFORMATION

't Report No.: 50-302/81-084/03L-0 Facility: Crystal River Unit 3 Report Date: January 25, 1982 Occurrence Date: December 28, 1981 Identification of Occurrence:

Concentrated Boric Acid System flowpath inoperable, contrary to Technical Specification 3.1.2.2.

Conditions Prior to Occurrence:

Mode 1 power operation (100%).

Description of Occurrence:

At 1800 while performing SP-300, Operating Daily Surveillance Log, Chemical Addition Pump 1-B would not pump. Chemical Ad-dition Pump 1-B was primed,-and operability was restored at 1820 on December 28, 1981.

Designation of Apparent Cause:

An engineering investigation is underway to determine the cause of this anomaly.

Analysis of Occurrence:

There was no effect upon the health or safety of the general public. Redunaant Chemical Addition Pump 1-A'was available and operable. No release of radioactive material was involved.

Corrective Action:

Corrective action to be determined following completion of an engineering evaluation.

Failure Data:

This is the fifth occurrence for Chemical Addition Pump 1-B, and this is the tenth event reported under this Specification.
