Containment isolation valves were inoperable contrary to Technical Specification 3.6.3.1.

Conditions Prior to Occurrence:

Mode 1 power operation (100%),

Description of Occurrence:

At 2045 while performing Surveillance Procedure SP-130, Containment Isolation Valve CFV-27 was discovered inopera-4 tive. Containment Isolation Valve CFV-27 was verified closed, and-maintenance was initiated. Operability was restored at 1445 on December 22, 1981. The same event occurred on December 23, 1981, for Containment Isolation Valves CFV-28 and MSV-148 at 0922, and Containment Isolation Valves MUV-41; CFV-26; and DWV-160 at 1340. These Containment Isolation Valves were closed and maintenance was initiated. Operability was restored on December 23, 1981, at 1130 and 1510 respectively.

Designation of Apparent Cause:

The cause of this event is attributed to the failure of the control relay for these valves.

Analysis of Occurrence:

There was no effect upon the health or safety of the general public.

Corrective Action:

The relays' coils and plungers were replaced and the functional

] test was satisfactory. These relays are functionally tested under our surveillance program, and they are being repaired under Modification Appreval Record 31-5-20 as repair parts be-come available.

This is the firt occurrence for these valves, and this is ,

the thirty-eighth report under this Specification.
